After re-watching the "E3 Demo (Part 2)," I have a theory about what is going on.  If you recall the video, you'll see that it looks like Sera, Iron Bull and Leliana get killed while Dorian and the Inquisitor are fooling with the amulet received from Alexius' corpse.  Since I don't think it's possible to lose two companions and one advisor in a mission, my guess is the portal that opens at the end is some kind of time warp that will undo those deaths and resolve the situation.

Prior to battling Alexius, the Inquisitor proclaims that she will "undo this," and Alexius laments that the past "cannot be undone" no matter how hard he's tried.  I'm guessing that the Inquisitor will succeed where Alexius failed.

This is a shot-in-the-dark guess, of course, but I'm anxious to see if anything in my guess is correct.  (And the matter of spoilers is really only relevant if I'm right about any of it.)  What do other people think?

Or it could just be like in Mass Effect 2 where you see your squad getting killed left and right if you did not help them out with their side missions. That is the likely scenario in my opinion  as companion deaths were confirmed as a feature to the game.

Dear God, I hope no time travel is involved.

I knew companions might leave, but I wasn't aware they could necessarily be killed.  However, it's the death of Leliana that really strikes me.  That would be a huge loss to the Inquisition, exponentially more than just losing a companion.  Essentially it would cripple the Inquisition by a third.

I'm hoping Bioware either kills companions or just doesn't. Not fake outs please.

I'm hoping every companion/advisor death is avoidable though, as long as you do things properly.

What you're saying totally makes sense from an in-world story point of view, but I was looking at it from a game design perspective.  I don't think the devs would be able to make an unlimited number of back ups to take over if you lost one of your primary advisers.
