I agree Ispan, it's so annoying they still didn't fix that push-to-talk bug.
My experiences mimics yours, if I am on a 3rd party voice platform like Ventrillo, then the game will pick up my conversations there, even though I have push to talk enabled, and even though I have different keys setup for talking in the various Places.
Lol, I've had my mic accidentally on when I've raged at rubberbanding or being stuck due to lag. It's the one thing that will infuriate me because it's out of my control. I charge into a group of mobs and forget to put barrier on myself? That's fine, my own fault. I try to Fade Step away from a group of mobs and get rubberbanded right back into them? RAGE! 
Promoted my Keeper and switched to my Elementalist for a few games. Usually have Firestorm on him, but some friends convinced me to try Firewall, so I did in a few Threatening games. I like it so far, I just need to work on my aim with it, nothing like missing the whole back row of archers, because I misjudge how thick the wall is 
This next story isn't my last DAMP pug, but rather my first one. For a little something different 
I started this game with a group of friends on the PC. We were excited for the multiplayer and when the game was released we played a few hours of it. Routine is easy now, but back then we were complete newbies and still figuring everything out, plus we didn't have the good gearz you get with time(and luck on your side.) We never managed to beat the bosses during our first session, but we did make our way to level 7.
My friends decided to take a break and I decided to keep playing. I decided I'll Pug a match, see how the other people new to DAMP are handling it. I get in a group with a level 1 Legionaire, Archer and Keeper. A nice balanced group, this will be fun.
Or so I thought.
We get in the map, Elven Ruins, and I've only taken two steps when the Keeper starts talking on voice chat.
"Holy crap! This keeper is already level 7!" He says, I decide to unmute my mic and reply.
"It's not a big deal you get that pretty fast." I say.
"Already? Friggin power levelers." The Legionaire complains.
"I got this after only a few hours and much deaths, it's not hard to get to." I state.
"Man kick this Keeper! He's going to go around one shotting everything!" The other Keeper says.
"Yeah guys like this ruin it for everyone." The Archer chimes in. At this point I'm a little taken aback.
"Trust me, I'm not going to be oneshotting anything." I reply, it was true, I was still in basically starter gear, no promotions and junk. "Look you'll see for yourself let's just...."
And I was back to the loading screen, because I was kicked.
My first few pugs were rough and it didn't help when my friends decided they didn't like the multiplayer and stopped playing it. Eventually I got some decent groups, but Xmas came and someone had bought me a copy of DA:I for the PS4 having no knowledge that I already had it for PC.
Switched to the PS4 as my PC is not the best and the game kept CTD'ing anyway. Haven't regretted it yet

Oh hey, you must've been in one of my games This is why I hate when I end up leader after a host migration... I seem to be a terrible host and spread the joy of lag like the plague.

Ugh, hopefully they fix that... eventually I'd really like to use my mic more often but I've already broadcast a conversation about Batman to a pug and probably more coughs/sneezes than I can count so I usually just disable it entirely.

Had a few host migrations last night, lucky for those in party it didn't land on my host.

If memory serves, that results pretty quickly in another host migration (that usually loses everyone in the ether.)
I had the pleasure of running with Courtnehh and Blacklitez this weekend. Blacklitez kept my Katari up in Perilous with his Keeper's uber barrier skillz, then Courtnehh and I ran a good number of times, switching off AW tanking duties. I must say, I've learned a lot and am becoming a better player while running with these guys. On a side note, Medallionx happened to wander randomly into a run of ours on his sick skillz lvl 1 hunter. We were in party chat so I never had the chance to find out what he was saying in game chat but I did notice his mike was going quite a bit. Apologies to him for that.

I can relate, I was put in a lobby on my own countless time when searching for Perilous games and when I finally did find a game I was promptly kicked to the dashboard...This happened twice and it took me almost 30 minutes to find a Perilous lobby. I'm becoming increasingly frustrated with the current matchmaking in this game - I'm tired of being paired with the same players constantly

In my last PuG, I was the stupid one 
I was changing my Necro's inventory and equipped my base Inquisition staff momentarily.
Later I've entered a Threatening match and I'm becoming aware that these enemies are rather hard to kill...
I still have my Inquisition staff equipped !
Wow you notice when your 150dps staff suddenly does 24dps...
I was incredibly helpful for the rest of the match, helping people kill things,
slapping Blizzards and impressive Walking Bombs on things...

Okay, what's Narnia?
Aside of the literary reference, it's when the map doesn't load properly and you find yourself on a plain surrounded by mountains.
It's pretty rare, but I believe it is increasingly becoming referred to as going to Narnia...
Very smooth, leveling up my Keeper in threatening, got a FR and turned it off for the night. Good pugs tonight, only had 2 fails, rest went fine.

It was my first (and, so far, only) trip, so I had to take a screenshot! LOL
Of all the luck, I entered Narnia just in time for Zone 5. I wasn't sure if I was any help at all, but I just kept trying to run where the mages were aiming and spam powers and randomly swing at thin air. We made it... somehow... probably only because it was a really solid group the four zones prior.
I have started playing alchemist almost all the time to try and get the banner and I must say, they hit like a truck. I've been outdamaging every class. I've had people change to level 20 dps classes to try and beat me and fail. If only I had the 200hp belt or HoK ring. I spent close to 400,000 gold this weekend and all I got was a walking bomb ring, a firestorm ring and a unique cooldown amulet.

Happened to me last night but daytime Narnia, should have taken a screenshot. Zone 5 and it happens, annoying for sure but it was just a routine run so I wasn't that salty, quit and re-booted my PS4.
The secret to entering Narnia is pulling a lever behind a breakable door right as someone opens the warp door to the next zone.
