Realistically? Well since I'm a surgical resident in real life I'd guess I'd probably be a private physician to some nobles/royalty, seeing as those were the few people who could probably employ doctors during the medieval period.  

 

That or I'd be some medicine man/shaman out in the sticks serving what ever remote village I resided in.
