I've lost my DA:O saves.
Does Keep allow me to create and name a custom hero from scratch? Or is it about picking up from a pile of premades?

You're basically recreating your world state with the choices The Warden and Hawke made, which "tells their story"
The different choices you made will have different outcomes in DA:I. Just like choices made in DA:O changed different side quests in DA2. But for DA:I your choices will have more impact than they did in DA2, if I understand it correctly

There are plans to let you rename the custom heroes (removing one of the syncing reasons)
