Yes, a lot of people have that problem, and there seem to be many possible different reasons for it.
If you've already tried all the solutions listed here, you might want to download the standalone DA Character Creator from somewhere like FilePlanet. You can recreate the look of your Warden with that, give him/her the same name and hit Upload. Some of us have found that works.
Just to be clear, though, you will have to recreate the decisions you made in-game by going to the Tapestry and answering all the questions there. This is the same, whether you manage to import your characters direct from the game or with the standalone Character Creator. The Keep never carries your saves over from your games.

My Wardens are appearing in the Keep, but there's no picture associated with them -- the space where their facial profile is supposed to appear is simply left blank. I'm concerned that if I try to upload these custom Wardens (who survived DA: O), Inquisition will crash when it tries to render them as they seem to have no appearance.
By contrast, my Hawke from DA2 (from the same console) appears just fine.
I can access my Hawke via the old forums (player profile diagnostics) and see the equipment she's wearing, but I can't see my DA: O characters on there.
I played DA: O and DA2 on the PS3, and I've tried the trick of going into my DA: O save files, messing with equipment, and saving the game over again. (I'm a bit tied up in terms of registering my DA: O game because I don't have a copy of the case the game came in.)
Any advice?

It's been confirmed that the Warden doesn't appear in DA:I, so the fact that their faces aren't appearing won't affect that game at all. Getting your Wardens to appear in the Keep is really just as a matter of record.
That said, their faces obviously should be appearing so, if you're just getting a blank where the portrait should be, I'd suggest you report that as a bug.

I finally got my new Warden to show up in the keep. Aside from waiting patiently i'm not sure if what i did had an effect.
After opening my network ports i disabled all mods and i created a new hard save(as opposed to quick saving which i used to force upload previously).

Fire up the game, you should be automatically logged in now (make sure by checking the bottom left corner in the main menu)
I have the ultimate edition and my game refuses to log me in. I am starting the game from within Origin, but I can't log in manually nor does Origin log me in automatically. I checked that my firewall is not blocking the game either.
Any solutions?

Okay, I finally got it to work.
My old Origin password had special characters like: "% ^ $ &" and that is why I couldn't log in.
After I changed my password I could log in manually.

Hmm strange and stranger. I have wardens in the Keep, but not on the BSN? And I'm still getting the invalid persona thing? And only 2 wardens actually have faces? And somehow a modded file got through? (Human noble mage)? Quite indeed strange and stranger. Even more confusing is that the Dragon Age servers are still down? At least for DA:II. And even stranger is I don't have the Ultimate Edition? Any advice here?
Even stranger still is that DA:O's servers work.

MY WARDENS FINALLY APPEARED I'M SO HAPPY.
I was legit kinda sad for days about it. I know that in itself probably says I need to get a life, but my Phoebus and Ailish were *mine*, and using Elissa and the default elf mage just didn't feel right. At all. Like, I know it's only a name, but they didn't feel like my Wardens.
However both have blank profile pictures, but both have custom hair so maybe thats it? I'm not even bothered, as long as I can remake them at some point in some future game (plus I'd want to do that anyway. Hawke can maybe get away with looking the same, but my Wardens (Ailish especially) in my head will have aged, and Ailish would have aged *badly* with dem wrinkles). But yeah, maybe it was just a waiting game and I'm so happy haha

I don't own the DLC (just the games), so now if I try to load my saved character, it boots me back to the title screen, listing the DLC I'm missing.
I'm not willing to purchase it all just to upload my character to Origin, but what can I do? As much as I'd love to buy DA:I, if I can't load my old characters & decisions, it's not worth a purchase for me.
I've been able to sync achievements to the Keep, but the characters are just blank. I can make a new save & new character, and that'll sync (so I know the link between my Origin and Xbox accounts are good to go), but am I out of luck for my DLC-packed characters?

Syncing your characters doesn't bring your choices from the games into the Keep. You always have to recreate those decisions in the Tapestry manually.
If you can't get your characters to show up (and I can't either) it won't affect DA:I. The Warden doesn't appear in DA:I and, although you can meet Hawke, it's been confirmed that you will be able to customise him/her in the game.

The only option available to you, really, is to start a new game, and create new characters. Which look identical, and have the same name...
Since syncing doesn't adjust choices in the Keep, and you can still make choices about the DLC in the Keep regardless of whether you owned it or not, this is a good option to take...

I was having a billion and one problems with my game never ever syncing to BSN and not even showing up on the diagnostics page, and I followed your instructions and my Wardens still don't show up on BSN but they are totally syncing with the Keep now that they show up on my profile diagnostics page.
That was a ridiculous sentence but I am very excited - this has been a pain in the butt for over two years!

Yup. The old saves were buggy so, although there are other advantages (cross-gen/cross platform etc), the main purpose of having the Keep is because save game importing just wasn't possible!
Creating your first World State can take a long time, especially if you have to keep referring to the Wiki as many of us have (it's not just you!) You may be interested to know that there a number of online guides to help you now as well, specifically designed for use with the Keep. There are others, but here's one from PC Gamer:

On the plus side, once you've created one, you'll find that creating a second and third World State becomes very quick and easy, so you can vary the details for subsequent playthroughs of DA:I in a matter of minutes.

Second I have probably a minor issue on this after I was able to synch my custom heroes in I notice it was only the heroes from my PC game and not any from my PS3 games so I created a world state from my PC heroes and exported it to DAI.
I pre-order the DAI however on PS4 it releases tomorrow, I pray I can still have access to my world state I created using PC custom heroes on PS4 DAI game.

Yes, it should be possible. Once you've created a World State in the Keep, the idea is that you'll be able to import it into whatever platform you're going to play DA:I on.
As I understand it, the game will prompt you for details of your EA Origin account (presumably email and password?) and from that it will find your World State.

I too am having problems syncing my Wardens. The Keep seems to only import the Wardens I have on my BSN DAO profile page, instead of the ones in my diagnostics page - deleting and saving again doesn't seem to fix this, sad to say. I can get the files to re-upload to the diagnostics page, but the characters I've deleted permanently (from my game as well as from the servers) still show up on my BSN profile page, and those are the ones getting synced to the Keep.

Ok so after realizing I would need the keep regardless that I have been playing on the same platform, PC, I went to the keep and my heroes aren't showing up. They are suppose to show up right when you get to the: Your Dragon Age Warden page and it says chose hero? I followed the instructions for both this thread and syncing your saves game to the keep thread and still nothing. How long does it take for the wardens to pop up in the keep? When I go to BSN it shows no data for DAO and I did every step. Made sure to delete mods and developer console. Updated game. Made sure I was logged in the game on the lower left part of screen and that data uploading is enabled. Reinstalled DAO in Origins as before I had the disc. Forced sync a dozen times even creating new saves. When I click online profile it takes me to BSN but it says there's no data even on the profile diagnostics page. Please help.

The Keep doesn't import things like romance options or decisions - they have to be set individually for each Warden and each Hawke via the Tapestry. (I had to figure this out too - just click on each image within the tapestry and set it the way you remember it.)
And it sounds like you're having the same issues with your Wardens that I am with my newer Wardens. Even the forced sync didn't seem to work for me as far as getting them to the Keep. (Although my One True Warden got synced, so I'm happy with that at least.)

Nothing is "imported" except the name and a tiny portrait of your Warden and/or Hawke.
If you want your World State to reflect the choices/decisions you made in DA:O and DA2, you have to recreate all those decisions manually in the Tapestry. This is always the case, whether your characters "synced" or not.

Is it not possible to register Origins anymore?
Can't get to diagnostic page because apparently I never registered it, and following the directions on the registration page just gives me "Code is not valid for game"
Also: Does anyone know if your characters' names will matter (e.g. showing up in codex entries or somesuch)? If not then I'm not even going to worry about getting the sync to work
EDIT: Nevermind, I remembered a workaround for the diagnostic page.
