I think what they need to do is to clarify whether the opposite of "friend" in this case is supposed to be "acquaintance" or "enemy." All they would need is to change the description to something like either "Cullen and Hawke were not close" or "Cullen and Hawke did not get along."

I think this is overthinking it a little. You can think of it as friendly acquaintance if you want. Cullen wasn't a companion after all.
It probably has very little bearing except for a possible moment in DAI when Cullen is asked about the Champion.
Friends: "Oh yes, the Champion. She was a good woman."
Enemies. "The Champion of Kirkwall and I were not... close."

Well yeah, my point is if you set Cullen as being "not friends" would that mean he'd say that they weren't close (like an acquaintance he doesn't know well) or would he talk about how much he hated Hawke's presence and influence on Kirkwall (like an enemy)?

Cullen had a very narrow "world-view" (when in DAO) we first met him. Depending upon you choices, you either bent to fit that view or expanded (your own and perhaps Cullen's world view)
Queue DA-2 and Cullen is still the same damaged, hurt human but.... Times change and so do people.

There was an optional (not quest related) conversation available with Cullen in, iIrc, act 2.
Unfortunately, I can't track down the exact text on the wiki, but the gist of one dialogue choice was 'stopping by to see an old friend' and the other choice was considerably less friendly.
AfaIk the conversation had no impact on any part of the game, but I suspect it's the source that this 'flag' would be coming from.

Since I'm not getting the game until Christmas, I'm just going to wait until it's clear how this tile affects his dialogue in Inquisition. If he's like "I knew Hawke... nice guy" or something, flick on the Friend option; if he's all "Hawke's totally like me and anti-mage" or something, then noooooooooooo...

Cullen friendship
If you're not certain what that tile means, rest assured that you're not the only one. No one's really sure what it means, but the general consensus around here is that it's more of a worldbuilding question aka: How do you want Cullen to speak about Hawke in DAI ?
At PAX Australia Mike Laidlaw said that this tile should have been taken out already, that it didn't make sense and that it wasn't tracked in DAI.
