First of all, why would anybody kill Leliana? Her accent is simple adorable, and she is so nice when she isn't murdering darkspawn. Secondly, even if you did kill her, she is alive in inquisition. If you did kill her in DAO, the canon story is that the warden only seriously wounded her.
Leliana has plot armor and bioware's favor. She would be alive in the middle of a blight.

Well, the keep asks if you killed her or not, but you know, she died on top of the sacred ashes, there must be some magic andrastian mumbo jumbo that explains her not dead state. You know, maybe not if you decapitated her, probably the events of combat finishers arent canon. Cant see how a pile of dust would connect a head and a body. Or, she's a zombie and the putrid smell of the war room wil escalete towards the end of DA:I.
the Keep says that Leliana was left to die in the heart of the temple. As has been explained in other threads the decapitation was just a gameplay mechanic that couldn't be disabled for single NPCs. You can also decap Arl Howe before he gasps his final words.

And in my first world state, she is dead in that as well. Since the way I see it, her "dying" can only enhance her story in inquisition. It can't take away, it can only add, therefor making it the better decision.
My world state is all about being interesting, obviously until I actually play Inquisition I can't rightly say what is or isn't the best decision, but I can take guesses on what I think will make the game more interesting. Another example is Loghaine, him being alive can only add content to the game.

I think them not following every single solitary decision and letting it effect the setting in a way that doesnt allow the world of thedas to be shaped how the writers want is perfectly understandable and acceptable. I would be annoyed if they tried to follow every decision they made possible, in each game, since you can really do some randomly dumb stuff. I think the fact you can do it in the game is reward enough. The setting comes 1st, imo.

That's silly, imo, since there were plenty of instances in that game that I decapitated an NPC that had to talk to me after I defeated them.

I didn't kill her. But then, I could never bring myself to defile someone's last resting place. It's just a personal line I won't cross. But I did make one completely messed up Keep run with all bad/evil choices and killed her then. Curious what that's going to do to the world in Inquisition, and what, if anything, Leliana has to say about being alive.
Personally, I think the thing simply was, the warden didn't finish her off or the Guardian just reformed after he left and healed Leliana. That temple was full of lyrium and true faith. Take your pick what the deciding factor was.
Regardless, she's alive, Lore > game mechanics.
I saved just before you make the choice picked the bad option to unlocked the specialization. Once I got it i reloaded and played the way i wanted. So I sorta killed leliana once but not in a proper playthrough.

It is a fairly common animation, especially if you're using a battleaxe as the stabby anims don't fire. I remember laughing my ass off when Oghren did it to Branka in one of my playthroughs.

Also was sort of common with duel wielding if the npc was right in front of you and not wearing heavy armor for some reason.

Leliana attacks you if you defile the ashes. It's on her if she ends up "dying" that way.
I did make a dwarf playthrough in which I just hated the world, like small man syndrome, but a bit more literal. Im pretty sure I killed her in that one.

This. You kill Leliana in self-defense. There is a huge difference. My cannon is romanced Leliana but had to kill Leliana and Wynne in self defense as they considered some dusty ashes more important than my warden's life and saving the world from the blight.
