Does the lifted target damage modifier work on a target that has been hit by Warp and exploded with some other power, say Throw? Or does the target need to be primed and not detonated?

AFAIK it only works with 'primed -not yet detonated' targets.
Or at least that statement always pops up somewhere...

I'm not sure. Damage over time is still active, even after the detonation. Whether it affects lifted damage or not remains to be seen.
Here is a video where I use Javik's Dark Channel and Liara's Warp in combined. A little hard to notice but it's clear that DoT stays after the detonation. See fighting Harvester at 2:29

DOTs always work for their full duration (except when the target dies before that time --excepted from this exception is DC)
But any 'lifted target modifiers' from Warp ammo or Cluster grenades (are there any more) only work on 'primed' targets. At least that is my understanding.
Addendum: I seem to remember a thread stating that the SP Warp-ammo modifier was bugged somehow.

I have heard that MP Warp ammo may be bugged, but I haven't heard about SP Warp ammo.

The "lifted target" bonus only applies if a target is biotically primed. If you detonate the BE, it isn't considered primed or lifted any longer, even if there are lingering DOT's.
It turns out this effect always worked, ambiguities in the old days were caused by the test methodology. There is a post by corlist somewhere showing that it works correctly in SP. And it definitely works in MP where the effect is pretty large with Warp 4 ammo.
