Sexuality is certainly a significant component of a lot of folks' identity and one that they'd like to be able to impart upon their characters, and I'd definitely imagine it to be in the top 5 if people voted on additional ways to shape Shepard. But Shep as a character is not entirely decided by the player by any means. Unless or until there is some sort of artificial intelligence going on behind the scenes in these games, there are always going to be finitely many pre-defined stories you can pick from that are basically all encased in one huge IF..THEN..ELSE statement. You can't, for instance, make Shepard deaf, pacifist, claustrophobic, pro-Reaper, or anything else on an endless list of possibilities that simply isn't included in the stories they have pre-cooked.
There's an end-goal that has to be reached and some Shepards (pro-Reaper or strict pacifist) can't work in Bioware's story.
Despite what some trolls would have you believe, being gay doesn't preclude men from fighting, leading, etc. in real life. The soldier I'm dating is proof of that. I can't think of a logical reason why we don't have gay Shepard except for fear of bad PR.
Ultimately, if Mass Effect 2 expects you to emotionally invest in your love interest so that the choices have weight, a gay player will miss out. That's all. Whether that's really a big deal is up for discussion.
