Just sounds to me like another, Isabella not coming back because she doesn't have enough friendship/rival points, or zevran siding with the crows in DAO if you didn't invest in his friendship.
What's this about a companion betraying the Inquisitor?

It kind of reads like if you get to the end with one companion, it's still possible to make the kinds of decisions that would get even that companion to betray you.
I wonder if that's the "demons win" ending?
Oh my god, combine that with the theory that it's Cole, and people thinking he's a demon, not a spirit and that would make perfect sense ![]()

honesly you may see Leliana (and Wynne) betraying you if you make the sacred impure! So they attack you. You need to become a good guy in order to make everyone happy!

I am not a fan of betrayl when it comes to companions. The issabela running off thing wasn't too bad since in the end it came down to whether she liked you enough whether it became a full betrayl. And anders didn't really betray you he just screws everything up. Betrayl is when a character does something to purposfully screw you over.

Yeah but they didn't really betray you. Morrigan kept a secret which was a crappy thing to do and she basically offers you a choice to live. It isn't exactly a great thing for her to do but it's not a real betrayl. Issabela's thing I liked since it made it matter whether you were close to her or not.
