Most of the SWTOR LI's mention either having kids or adopting them, but it doesn't actually happen in-game.
Yes.
I have no problem with having a talk by the campfire about what my character and their LI plan to do after the crisis is over. As long as there are different options. - We can watch our kids from the front porch, We can travel to your homeland, we can help the chantry, templars, city rebuild, We can see where the future leads us. - But having kids in a crisis situation and finding a wet nurse, is not something I want to participate in.
I have adopted kids in Skyrim and really, I don't go home that often, but also the story in Skyrim doesn't have to be followed so my bard just does a lot of traveling and occasionally kills a dragon then goes home to see the hubby and kids and then goes out again.
DA2 would have actually made a little sense since it covered a number of years. Glad they didn't though.

I think having a conversation with LI about such things could open a whole different can of worms.
"Darling, do you want children?" - "No."
Then what? We break off our romance? Or try to force him/her to change his/her mind? What if he/she doesn't? Then a lot of topics in BSN popping up "Why Cullen/Cassandra doesn't want my babies?"
A lot safer to leave that to headcanon, in my opinion.
Headcanon is my choice. If they did do something like this in a future game, I think it would have to be an option brought up by your character and if it's brought up, the LI has to LOVE the idea. Or people will be very unhappy that their LI doesn't want the pitter patter of little feet running around. It is not an option I'd pick. Not in a game like this.
