Actually I don't think anyone knows. But if you want to speculate: I'd say something like Corypheus (from DA2: Legacy) but more powerful. Since it's the fade, maybe some how invloved with the Black City. In the promo vid it did mention that the Maker is dead. Maybe it is the Maker's evil side?
Remember, the Golden City was already black when the Tevinter mages entered. Corypheus states that. So maybe something else corrupted the city and this is the end result.
