Unstoppable Achievement Help?

In DA2 there is an achievement where you have to complete a full year in Kirkwall without any party member being knocked unconscious. 

I feel like I might have done this before when trying to get this achievement, but I could of missed something. 

I was wondering if anyone knew the following about this achievement....

 

Do you have to play on a certain difficulty?

Is there a certain amount of time you need to spend playing/in combat/quest completed to get the achievement? 

 

I know that a year means an Act. I know what being knocked out means. I don't know if the answers to these questions will help but they might so if anyone knows this I would very much appreciate the help. :)

 

Also sorry if there is already a thread of this. I searched but couldn't find any.

I went through an entire act on Casual and got this achievement, so I'm pretty sure you get it regardless of difficulty. I'm also pretty sure that there's no timer on anything. You just get it if you make it through all the combat without someone falling.

 

OK, I just checked and the only other limitation is that this does not apply to Act 3, so it must only be Acts 1 and 2 that you can get this achievement, if that helps.

Apparently, you can still get the achievement if you reload to a point before you or a companion falls in combat, but that just kinda takes the fun out of getting the achievement.

Here's a description that may help. Its for the ps3, so I'm sure its the same for an Xbox

 

For this trophy, you must survive an entire year in Kirkwall without any of your Companions going into "Pimp Down" Status. While this may sound like it is difficult I can assure you that with the right preparation you will find that this is a very easy trophy. Follow these tips below to help assist you in this and as soon as you finish a complete year in Kirkwall without any member of your posse going into "down" status the trophy will unlock.
 
Complete this by the end of Year 1: This is be far the easiest of the playthroughs to complete this on. Your party doesn't start out very powerful but by the end your team will be fairly strong in battle. In Year 2 you have several difficult enemies to battle including Xebenkeck, Varterral, and the Arishok so that is out. In Year 3 your team will be very powerful BUT you have to battle the High Dragon, Meredith and Orsino. In Year 1 the extent of your boss battles is the Demon in the Deep Roads and a couple of Mature Dragons who aren't overly difficult so I definitely recommend this Year as your target.
 
Always have a Healer: In year 1 you will have Bethany in your crew, unless you are a mage, and you will also have Anders as long as you go grab him. Both Anders and Bethany have access to healing spells and at least one of them should be kept in your party at all times. You cold even bring both if you are overly paranoid about it.
 
Properly set up your team: It doesn't do you any good to just toss Bethany or Anders into your posse if you don't change up their tactics. Head into the Characters Tactics screen and set whoever is going to be in your party as the Healer to these commands as their first two actions:
Party Member> Less than 50% Health> Heal
Self> Less than 50% Health> Heal

I did this on Casual and I'm currently attempting to do it again on Nightmare, just as a self-imposed challenge.

 

No revives, restarting the fight whenever any character gets KO-ed and using only potions I loot from dead bodies.
