Harvester was the longest, arguable that it was the hardest.

Whats the hardest fight in dragon age so far? I eventually played both games and all dlc on nightmare but there was 1 fight i had to go to normal on. The hardest ones i remember were: Gaxkang, Da2 high dragon, flemeth the shapeshifter, harvester agmarrak, and that damned first brood mother.
But the only one I could never beat on nightmare was Corypheus. Every step was an AoE disaster
Ancient Rock Wraith for me, don't think I ever managed to beat it on Nightmare without reducing difficulty. Certainly at least it was the most annoying enemy.
Even the toughest boss fights in DA:O on Nightmare setting were a cakewalk compared to this one. In Origins you can generally spam heals and cheap healing potions to get through any boss battle.

Malvernis is pretty high on that list, but I would give my No. 1 toughest fight pick to the Arwegian Scrolls: Second Aspect sidequest. Two boss-level revenants. I basically had to cheat my way through that fight.

Ser Cauthreian just because it was clear that it was a "supposed to lose" fight that you can actually win. She just puts out so much damage and the number of enemies that attack is so unfair that if you don't actively know it is coming or haven't gone some ungodly spec then there is little you can do.
Actually never mind, it is the optional golem fight from Golems of Amgarrak. That fight sucked. Several Elite and Boss level Golems that will throw rocks at you every chance they get.

I still find the ruined temple high dragon to be a major pain in the ass, more so than the one at the Bone Pit, despite the minions. Constantly flying away, landing and then sucking everyone within buffet range, fireballs that actually homed in on your position, and constantly throws everyone back, while you and your group take just long enough to get back up that the dragon flies away again and shoots more fireballs from a distance.

Flemeth is easier by default since the little clearing by the hut basically stifles any mobility. It's not quite a fish in a barrel, but it's about as close as you'd get compared to the other dragons you fight anywhere else. I'm not sure if Flemeth's health is on par with the other high dragon, but somehow it just feels like it's less.

But the Harvester? Sweet mother of god, that was a horror. On Nightmare, @ level 35 I was totally caught with my pants down and never expected how bloody hard it would be. The first time, to my shame, was a total team wipe.
Did better second time around and won, but bloody hell.

For context, Aveline has been nearly invincible throughout the vanilla game and the Legacy DLC during that playthrough.

flemmeth is easy if just for the fact that you can cheese your way through the fight. two archers and wynn holding position at the bottom of the clearing and a tank in fire resist armor holding her attention from the hind legs so she's constantly spinning and trying to attack you

For context, Aveline has been nearly invincible throughout the vanilla game and the Legacy DLC during that playthrough.
That Arcane Horror was defo a pain, but he is managable.
The problem with it is the damaging AOE he unleashes, which basically makes melee characters uselss against him.
I personally used my Melee Hawke to simply smash mobs or bait him into unleashing his AOE whilst running out of its range. Dispel magic + range was used to whittle down it's health.
But yes, that was difficult.

Alpha Wyvern was one of the toughest fights and the most annoying. Because of its ridiculous damage and combo speeds I have to pause every 2 seconds to reasses its attack pattern and issue new orders. Leopold was a cakewalk compared to that.
Other than that - Malvernis in Legacy, the Sky Horror in MotA, some encounters (mostly in MotA) with lots and lots and lots of archers. Xebenkeck and the Awiergan Scrolls' two Revenants are hard if you try to do them without kiting.
In DAO/DAA there was only one tough fight - the Harvester. The rest is easy after you've played the game once.

Vanilla is Xebenkeck in DA2 if you don't get fire resist runes...tied with the two MotA bosses.
Sky Horror and Alpha Wyverns are RNG-fests, each boss having an ability that one shot people out of nowhere, so they are less about "difficulty" more about "dumb luck"

This, at least if you fight it straight up. It's pretty easy to exploit if you train guys back and fight them in one of the doorways, but I'm pretty sure you can exploit every fight in the series.

Yeah I was talking about straight-up no-exploit difficulty.
All fights in DAO can be exploited. Less so in DA2 but still possible.

No.
SnS Reaver works just fine. 2-Handed Reaver could also be fine if you had him specced for spike damage and CCC.
You get seriously damaged by the spirit AOE -> activate Sacrificial Frenzy -> unleash all your spike damage talents and CCCs. The bastard loses so much health he has to teleport out. Rinse and repeat 3 (or was it 4?) times while managing the add-ins.
Still, you have to be very precise.
