Chrome. I finally got it to work by tinkering with my anti-virus/anti-malware program. I had to reload the Keep after I did this (it wasn't allowing me to make changes even after I shut off the AV/AM program until I reloaded the Keep).

I had the same problem with Chrome, so I use IE.
At any rate, I'm trying to make changes to the tapestry (I downloaded the "default world state"), but I'm not getting the "Approve this choice" for all of my choices, only for some. So, for example, it's insisting that my Warden Queen died killing the archdemon and there doesn't seem to be a way to change that.
You may have already tried this (in which case I apologise for stating the obvious!) but check your tiles for Morrigan and whether she had an Old God Baby (i.e. did the Dark Ritual / had sex with Alistair or Loghain). If she didn't, then whichever Warden killed the Archdemon will die.
