Does anyone know what some of the cross-class combos are?

As I understand it, playing on harder difficulty requires that you have a good grasp on exploiting "weakness" conditions imposed on enemies by your party.  The most obvious one that I've seen, because it happens a lot, is when I hit a frozen enemy with a powerful longshot, and get the "shattered" notification.

 

What others should I be looking for?  Cassandra tends to "sunder" enemies, so are there specific attacks that I should be using to follow up on that?  Or in most cases, do I simply need to prioritize any enemy in a weakened state, so that I can bring them down more quickly?

 

When something gets classified as "weakened", what's a good way to exploit it?

I usually use Solas to freeze enemies and then use exploding shot/long shot with varric, or a heavy hitting attack from my rogue.

Also think when enemies are knocked down incoming attacks either do more damage to them or ignore a portion of their armor because I notice my ticks are higher against an enemy who got knocked down.
