After many hours of no sleep, I somehow decided that DAI should have Dark Souls' control scheme. Surprisingly, it worked very well for my warrior tank main on Nightmare difficulty, and boss-only tactical cam & pausing.
Note that to get this fully working, you will need a gaming mouse that supports rebinding/macroing mouse buttons.
I won't go over the basics of AutoHotKey. Google it up if you are not familiar. AHK script below:

Further, I changed the binding of my right mouse button from RButton to "8" on my SteelSeries Sensei.
In my game, my ability 8 is Shield Wall (or Parry, or a similar ability), ability 7 is a gap closer like free guard points charge. Space bar is bound to a dodge ability.
Press "Y" or mouse side button 1 and the camera will be toggled into a free cam mode you'd see in regular TPS games. Enjoy glorious left click attack and right click block/parry. Time the block/parry carefully, just like in Dark Souls if you hold up your shield all the time, you will simply run out of stamina and contribute no damage. The free cam also makes orienting the character much easier, allowing you to easily block attacks from multiple directions, as long as those attacks are not synchronized.
I haven't tried the setup in multiplayer yet, but in theory it should work very well as it has no reliance on tactical cam or pausing.
