Here's a list of the problems I've encountered, I tried to be detailed as possible.
The one that really blows my mind though is Sera's back flip ability where she ends up backflipping across the map or up into the sky 
PC: ASUS RoG Notebook (GTX 670 CPU 2.4 w/ boost to 3.0, Win 7, 8G RAM etc - meet all the requirements to run it on HIGH / ULTRA) I haven't left Haven for Skyhold yet but this is what I have come across so far.
Wartable Map / World Map / Region Map:
- Sometimes the items on the map will become unselectable by mouse, I have to use keyboard arrows to move around the menu or I have to force an ALT-TAB out of screen to get it to work again (which then leads to other problematic issues.) This also seems to affect all menues in the game (Inventory, crafting, etc). This is particularly stressing on the Wartable Map.
Dialogue Wheel / Conversations:
- Sometime the PC UI will not go away during cutscenes (especially predominant during the conversation with Connor in Redcliffe). I have to re-load to make it go away.
- Sometimes the grey circle of the Dialogue Wheel will remain on the screen with blank options after I speak with someone instead of disapearing which will then lead to not being able to select any options to get out of the conversation (NPCs in villages only, hasn't happened with my companions or the Advisors).
- Sometimes the mouse will stop working and I am forced to use 1,2,3,4,5,6 to choose dialogue options. ALT-TAB seems to be the current work around.
- Sometimes, I will see Speech Bubbles floating around with no NPC around, or the speech bubble will be above my ability selections on PC UI with no option to make it go away (this usually means that the dialogue wheel will show up grey with no text and won't go away, have to reload.)
- Sometimes I can move my camera around when speaking with minor NPCs or ambiant dialogue but now I'm always stuck in one direction, I can't spin the camera anymore (I'm not talking about cutscenes).
- The zooming thing it does for conversations isn't very smart. Depending on how my character is facing the NPC, I get caught up behind walls and vegetations / pots / counters all the time with no way to get out of it unless I skip the conversation. (IE: While speaking with Cullen outside, the NPCs fighting around him will block the way or all I will see are the swords)
Music, sound effects:
- After loading from a save, the sound effects of battle do not play or will have a very long delay (10 seconds +) which will then play all the sounds at once to get caught up.
- Sometimes I've encountered load screen menu music overlapping with zone music which is really hard on the head.
Companion, Battle Issues:
- Sera, while using the leaping arrow ability on an edge or a hill, will either fly up into the sky (a la giant in skyrim) or flip half way through the map, which is really funny the first time but really irritating after 50 times. No work around that I have found besides "make sure she's not on a hill or an edge"
- Companions will not use newly mapped abilities. Have to reload.
- Tactic Cam seems to get stuck in one direction sometimes and I can't turn properly on the keyboard/mouse, this issue doesn't happen when I use my controller.
- The locking on Target isn't easily accessible on M/K, I wish there was an auto-select to next aggressive mob. It's really irritating on a melee or tank character to be constantly fiddling with 12345678, ASWD, R L/B + Lock on Target. I need more hands, obviously.
Collision Issues:
- Partially falling through the floors / hills / steps. My feet keep getting swallowed up by the ground during big cutscenes.
- Some items are floating in weird spots (like a fire pit with people around it in the middle of river water, Inquisition Loot Troops that walk around collecting herbs are stationed in the middle of a lake that I can't get too? They're Jesus, they can walk on water.)
Other weird things:
- Companions or ambiance NPCs will seemingly Superman Fly Land into the immediate area I am in or in the background while speaking with NPCs. Especially hilarious during cutscenes.
- NPC flies out the closed gate in Hinterlands as you are speaking with the gate person(where the Apostate Mages are holding up - Where the quest for that Elf with the breathing potion is)
- Textures only partially loading, part of an outfit will be crisp and then the rest will be weird blurry and blotchy.
- The camera focus will be in the background in major cutscenes and all the characters in the sceen will be out of focus (ie: After fighting the Envy Demon in the Templar's story line (Champions of the Just I think it's called?) and you walk back up the stairs toward the Good Templars, it's all blurry). I'm really happy about that wall texture but everyone that is up close and personal for this amazing story developing is extra blurry!
- NPCs slide everywhere.
- Would be nice if the party would stop freaking out while I'm talking to a NPC, Cassandra really needs to stop pacing around lol.
- Origins knocks me out of the game if someone sends me a message. this is really jarring and irritating.
- Party members will knock me out of a dialogue wheel or out of an NPC's conversation range while they're pacing around. I've had to resort to telling my party to hold in a further area before I speak to anyone.
Better Life Items:
- Being able to back out of party selection if you chose to "venture forth" when the selection bugs out with the mouse or if you realise you forgot to sell all your stuff at the war table.
- Auto-attack needs to be included and be optional to turn on or off. At least let us be a ble to bind attack on something else than left mouse / R. Playing melee is really irritating without auto-attack (hitting 1-8, ASWD and pressing down on the mouse button or R to attack, I don't have three hands and I hate using the controller.)
- Looting is irritable for several reasons - you need to be really close, you have to spam V since it won't stay highlighted after you discoverd it and loot all is not bindable.
- I find myself pressing down on both L/R buttons on my mouse to move forward like in the other games, it would be nice to see it added back in.
- I don't mind the search function, but I think once something is highlighted, it should glitter or something so we can find it instead of spinning the camera around spamming V to see where the beep might lead us. The arrow should be a permanent feature, not a "important hidden item" feature.
- We need a Skyhold coffer or something to put extra items in. Pack Rats matter.
I'm sure I'll find more, it's not as buggy as ME3 anyways! 
Besides this, the game is fantastic, 25 hours in, haven't even left Hinterlands lol. Awesome.
