For some reason you didn't include having Carver and Bethany alive. Why ?
You should be able to. How ? By letting Bethany go to circle of magi and carver joining Wardens.

Nope, that's not possible.

 

If Hawke is a mage, Bethany will ALWAYS die in the ogre fight.

If Hawke is a rogue or warrior, Carver will ALWAYS die in that fight.

 

It's not possible to have both alive.

I gotta check this, You might be right tho, my memory is just not good.
When i was recalling moments i only remembered on begining that Avelines husband died.

Altho Bioware could put romance with Anora -,-

Incidentally, I think there was a mod floating around somewhere that allowed both Bethany AND Carver to live, or at least to switch one for the other. Perhaps you remember a playthrough with that? In any case, what I wrote above is canon, anything else is wishful thinking

I don't remember now, maybe i somehow cheated and made them alive . I just didn't like that
