In my DA Keep-->DA II-->Companions--.Chocies Bethany because I have only three window:
Bethany did not live to see mag/templar war.
Bethany did not die in final battle
Bethany died in final battle

I use DA Keep in Polsih language whether it is possible a mistake in translation?

Why is "Bethany did not die in final battle" not the choice you are looking for?
My english is week. But In polish translation maybe is a mistake because in this polish translation line that you quote Bethany also dies.
Polish translation looks that:
Bethany nie dożyła ostatecznej bitwy so in english this looks more or less that: Bethany did not live to see the final battle.
