Not sure.  I play on Xbox 360 and fortunately do not have the disc unreadable error.  I just get the pop-up, some sound issues, camera no longer auto-centering on character you switch to (REALLY annoying during combat), less loot after Patch 5, etc.  IIRC, my Xbox 360 was manufactured sometime in late 2012 and in November 2014 I upgraded to a 500 GB HDD.  The only thing I can suggest is to delete the game data from the 360 HDD, but not your save game data obviously, and let the game data re-install.  There could be bad sectors on the 360 HDD, where your DAI data is currently located.  If that solves the problem for DAI, great.  If not, then I honestly don't know what to tell you.

 

Incidentally, have you contacted Bioware to...oh, wait, nevermind...For a second I forgot they pretend we don't exist...

We don't, I'm just a voice in your head... a voice who has tried reinstalling, etc, etc... I was thinking of just waiting till they bring the game out on new gen and getting a new console but i've been hearing recently that since the new patches that game is just a figment of our imaginations too. Spooky...

Well, we just have to wait with patience until patch 8 come! He will (perhaps) fix this bugfest.

 

After all, we must wait until the game runs perfectly for the PC and the next-gen consoles before they start to work for their old-gens fans. It's not because we paid the same amount of money that we must hope they'll take care of us.

I had a similar thing happen after I finished Dorian's quest. The credits music just started blasting.

Same here. I even get the war table music in middle of fights.

 

Edit : there is news about a new patch for the xbox consoles. Be careful...

But this is the GOTY! I think they think they have got away with this... that they made a product that works for most people and is just shoddy for the few. I think factoring in all the people for whom this wasn't game breaking but it was still a bit crap then that few is nearer to say 5% than 1% of their fanbase. I think 5% of their fanbase could still spend some time telling people how much it sucks. Personally I am suffering a little from fatigue. They don't even bother to answer my twitters/facebook messages any more. Has everyone else stopped bugging them? If I'm one of a few lone voices then they'll just totally ignore us, instead of pretty much ignoring us...

Firstly, sorry for the belated response, posting this from my mobile since my Internet has been down.
Tis a sad state of affairs when such a broken game has been declared GOTY. I am still persistently sending them messages on Facebook, I completely forgot to check my Twitter the last few weeks, but so far I have had zero responses outside of this forum. The more this goes onward unanswered, the more I don't want to give it up though, purely out of anger and frustration. That is for sure. 5% would definitely be able to make an impact on the reception IMO, but EAware is like a steel wall when it comes to ignoring their consumers and completely blocking/ignoring bad critiques.
Definitely fatigued, but still here.

I know Patch 6 is supposed to fix one of the mosaic pieces. Anyone know yet if Patch 6 made things worse for the Xbox 360?  I want to know if it's better to keep playing offline or not.

if so, why didn't my game patch? Yes, I have the PS3 connected to the internet, yes I was logged into both my EA account and my PSN account, yes my PS3 is up to date. The game didn't patch. And I exited out of the game 3 times to try and get it to patch, and NOTHING.

I haven't tried the PS3 yet. But husband said the patch definitely ran on PS4.
