I want to know how much game change when i make decisions in "Keep"? Like when i choose to kill some characters or not, or make Alistair to king or not.

You may lose companions under certain decisions in the previous games, especially DA2. The gameplay doesn't get affected except for some minor information being reviewed. Some characters you choose to kill will not appear and you may lose some support arising from keeping them alive.
