If you start a New game,
IN the prologue
BEFORE you talked to Leiliana at the table with the Bureaurcrat guy.
and AFTER the game tutorial actually Unlocked your character record...
Check your AUTOSAVE files. Should be 2 of the most recent.
The first one loads you in front of the gate guarded by the two archers. With character Record Unlocked
the Second one ALSO loads you in the same spot. BUT CHARACTER RECORD IS LOCKED.
Check again the Autosave, Look at the TIMESTAMP. It might be the cause. saved on a different timestamp, it confuses the unlock progression with the second autosave thinking you HAVE NOT progressed the tutorial as far as unlocking character record, but the GAME progression has PASSED it. Thus it never actually going to be unlocked!
