Yes, there were a few discussions about that in the Keep. She was alive in DA2 as well. In one of the DA2 add-on packs she says something vague like "the Maker decided it wasn't my time", but there hasn't been a satisfactory explanation yet.
I'm guessing that maybe more will be revealed as you get into the characters' backstories? Give it time!

They decided to give her a larger role after Origins, so they retconned her back to life in DA:2. I am disappointed that they had to update DA:2 to give even a little explanation, but at least it's something. I hope the final answer is something good. I really liked the sacred ashes quest and the Guardian.
