The initial flirty lines are actually really subtle and nice: you call cassandra a force of nature, and when cullen cuts himself off with an "i'm sorry, you're not here to hear lectures" after he starts getting excited about the conversation, the flirty option is just saying that you wouldn't mind listening to whathe has to say. it's not until skyhold that they start to get more obviously romantic, i think. or at least more direct, and by then a lot of time will have passed.

Cassandra makes it very clear that she doesn't care about your flattery lines. She just gets annoyed, because you didn't build a relationship, and she's not the flirty type.
So, yeah, I think it's good that BW allows us to try stupid things and then having our companions call us out on those stupid things.

Well, seeing how from Orzamar to Fereldan's Magi circle, is 2 weeks and... 4 days? minimum. So, from Haven (same mountain range as Orzamar), to the hinterlands would likely be 3 weeks and 2days. 6 weeks if including a return. So, you spend at least a month to two, just doing one mission. Orlais' capital (forget spelling) would be at least a month and a half to. So for us, it's been an hour, in game it's been like 6 months. So, it's only natural to start flirting.

Haven's in the same mountain range as Orzammar, but it's significantly further south. Redcliffe is also on the "near" side of Lake Calenhad for Haven, as compared to Kinloch Hold which is on the "far" side. They're just not similar or comparable situations.
Travel time is also subject to the vagaries of circumstance. We don't know the exact distances involved, we don't know the conditions of the country between those locations, we don't know about any potential weather events that would've caused delays. So even presuming your number for Orzammar-to-Kinloch Hold can be sourced (and I dunno if it can or not) it's not going to be a number that can be generalized. And it's certainly not a number that can be generalized and then used as a basis for comparison to a thing we know nothing about.
I think that this is, in general, a sound plan on the part of the writers. For the most part, they haven't given specific absolute numbers about travel times, distances, or any number of things about the world of Thedas. That way, they can't be wrong. So long as they keep within a fairly wide band of time for "stuff" to happen, they're reasonably safe. And they don't need to give specific information about travel times or army sizes or whatever to keep the world an immersive and plausible one.
But let that go. Even if I don't agree that we can pull specific numbers out of a hat, I do agree with you that a lot of time passes. I think that it's reasonably clear that the game involves significant time skips.
I think that despite a lack of specifics, we're still obviously talking about a matter of days and weeks, not the minutes and hours that the player sees.
And, y'know, minutes and hours are still pretty believable, at least to me.

Can't say anything about the first flirts with Cullen or Cassandra, but I took the option to flirt with Josephine upon her mentioning my parents, and my response clearly caught her off-guard as silly and irrelevant to what she was trying ask me about. I felt like a lame idiot, and quickly reloaded.

I went back to the Haven, recently. After splunking in the Hinterlands, quite frankly we got bored, it was the first time I even talked to Cassandra and Bioware thinks it okay for me to just hit on her?
Really?
Last time I checked this ain't how romances go.
Except it is not the first conversation you have. Nor the second. Nor the third. Nor the fourth.
Despite that it would feel very out of character to flirt with another party member. Let alone the first ever conversation we ever had.

I thought one of the first flirts you could do with Cullen was quite good actual, the whole vows of celibacy thing. It seemed to me like my Inquisitor was really just interested to know if the Templars had vows of celibacy, and Cullen just took it way out of context and got super embarrassed. But you could also read it as the Inquisitor purposefully asking the question to get a reaction out of him of course. I had mixed feelings about Solas' flirts. Some of them where nice, like Solas' 'all my friends are spirits' one, but the 'Hey I just met you and this is crazy but I'LL PROTECT YOU FROM THE TEMPLARS AND SAVE YOU, BABY' seemed a bit... abrupt.

I must admit the 1st few flirt options with Cass did feel not really appropriate in the context of the dialogs during which they were presented, kind of like they were put there as an afterthought, like "hey, we forgot to make flirt options with Cass in the early part of the game, it's 3 days till release so let's do them".

Nah she was just surprised. Of course she disregarded it, the Inquisitor is basically the Jesus of Thedas, and there he was, trying to hit on her. It was quite subtle anyway, it could have come off as a joke.

I took that as a 'you saved my life so I owe you one, besides, you're trying to help' kind of protectiveness of Solas. The other nice option you can respond with is saying Cassandra will protect him. Same thing really.

How can people misinterpret the massage??? Especially by Zevran! Lol, don't accept massages unless it's a doctor's prescription. Didn't you listen to what mommy said about accepting candy from strangers? It never ends well.
Seriously though, it's annoying to witness this cycle of whining over and over. People don't like something, Bioware is so stupid and tries to please their fans, but then they don't like those changes. Bioware tries to do better next time still. But it's no use.
What pisses me off the most about it is not that fans are never happy, it's that they accuse Bioware of not listening to their fanbase! LOL, what?!
I've been on BSN since ME2. And every game Bioware tried to address most of the complaints. Whether or not the changes were for the better is beside the point. But they do listen.
People hated the insane amount of loot in ME1. They got rid of it. ME2 was TOO streamlined, so ME3 had more RPG elements again. People hated the Mako. OK, gone. But oh no, the planet scanning was even worse.
DAO combat was too slow. In DA2 it was too fast... DA2 had tiny recycled maps. DAI is now too big.
The problem is that Bioware always goes overboard trying to do the opposite if what fans complained about. That is certainly something they need to work on.
Personally, I would suggest they just follow their vision of what game they want to make and not listen to all the complainers. No game will please everyone. I don't understand why they even try. The result of their efforts to pander to every request can leave everyone unhappy. It's never a good idea to censor your work in order not to offend anybody.
Coming back to BSN reminded me why I left after ME3. I loved that game. And I love DAI so far. Gamers are the worst complainers I have ever met.

That said I did hate the stupid heart symbol in DA2 that we only got because of Bioware's misguided fan service.
However, I thought it's possible to turn the icons off in DAI? I haven't tried it yet actually, but I remember seeing a setting for it. I kept it on because I was curious about what kind of icons there are now and how they're different. I'll play around with the settings tonight.
If it's in fact possible to turn them off, I don't understand what the problem is.
Compared to male Shepard's creepiness, DAI flirting is fine to me. I actually like being able to flirt with a companion who's not even a LI. It's not exact creepy if it's more of a tease. I have only just met Dorian but I like the idea of being able to flirt with him as a woman and him actually enjoying it. That's awesome. Flirting does not necessarily mean sexual attraction. I mock flirt with my female friends all the time (I'm female myself!). It's just another display of affection.
Of course how it's done is key. And when I clicked on the heart with Vivienne I was kind of shocked my character was so suggestive! O_O oh man, how much fun it could have been to have flirty banter with Viv! Neither being serious. Oh well.
There is a LOT of room for improvement. I liked DAO best too in that respect. I'm not that far into the game yet. Haven't hooked up with anybody. So I can't say how organic it feels to me. But the possibility to flirt is not an issue for me. On the contrary, I appreciate the choice. But it better be classy!
I'd like Bioware to play a bit more with the sexuality of their characters. They already did a great job writing two butch female companions who are straight (Aveline and now Cass). And this time the female romance options aren't that attractive either. Bold move. They are catering to different orientations and preferences more than ever, and I appreciate it. I'm a straight woman, but I'm always happy for those who feel underrepresented. I was happy for gay Kaidan fans to get him as a LI in ME3. Didn't ruin Kaidan for me at all. He's straight in my femShep romance. All good.
I wish I could romance Dorian as a female inquisitor. Would be funny to have a fabulous character who's NOT gay. My dad gets mistaken for gay because of the dramatic way in which he talks.
Anyway, I don't mind the option to flirt with everyone. It doesn't break immersion for me. I pick dialogue options that fit my character. I get the point though that it sucks a bit when you want to play reserved and shy but the flirt option is heavy-handed. Jacob in ME2 comes to mind. I almost stopped talking to him because femShep talked as if she wanted to rape him. Worst thing about the entire game. What were they thinking??? So as long as there is no such creepiness in DAI, I'm good.

I too felt a little creepy when I saw the flirt option when you first meet a character. It concerned me because its a video game, and I'm not sure what will happen if I do *not* choose the flirt option. What if I wanted to get with Cassandra and if I missed the first flirt then its closed off?
So I ended up being a little creepy and flirting with everyone right off the bat.

My roomie flirted with everyone her race/gender couldn't get in addition to her chosen LI, it just comes off as really flirty, not really awkward
