How do I get Cassandra (or whoever) to hold position but draw aggro? I know she has a couple of abilities that will draw aggro, but is that something I need to micromanage? If I let the battle play out in real time, the enemies usually stream right past her while she stands around, usually facing an entirely wrong direction.

Challenge is a ranged taunt, but its only single-target. The AI will actually use challenge on its own even if when they're holding position.
For the AoE taunt, the safest bet is to probably switch to her and use it manually when the enemies move within range. I bet you can have the AI do this automatically, but then you need to consider how much you trust the AI.
If you do use it manually, remember to use the hold position command afterward if you don't want her to move from her spot.

Nope. You can rely on Cassandra running in first then using war cry because that's how it's supposed to work, but again that depends on whether you trust AI, while sometimes AI in the game is actually pretty good, it does become dumb on several occasions
You can always just use tactical camera to position her then use war cry, will even give you a chance to examine your enemies.

Set her to Follow herself. Make sure you have abilities like Charging Bull, that vastly reposition her, disabled. At the start of a fight run her to the middle of the most dense mob then Warcry. Make sure Challenge and Warcry are on preferred. She'll continue to taunt, building ridiculous amounts of guard till everything around her is dead. Set your DPS characters to follow her. Set a Barrier mage to defend her. She'll be nigh invulnerable.
Hot damn, im gonna do that with my own warrior who "was" 2-handed and will now be S&S!. Thanks for the tip there. xD
