Many of the glitches I've encountered are already covered. I'll try not to be redundant. Pictures will aid in my descriptions for some of these.
1. Inquisition found me a chest... that I can't access... in the middle of the long river. I have no idea why this happened.

2. Some cutscenes don't load animations. There's this hilarious goat scene that plays out when this loads improperly. One time, a scene like this ended and my PC's animations didn't load during play, causing him to slide (rather than run) and jump while frozen in that body shop position. I do not know the trigger of this issue. Apologies for not having pictures of this hilarious glitch.

3. Unfortunately, this is another visual glitch I cannot explain, but it's pretty funny looking.

4. This is easy to reproduce. Climb a box, a cliff, or any location not deemed a "safe zone" by the game. Enter strategy camera mode and the game will pause and simultaneously teleport you to the nearest place it deems safe. This causes any clothing physics to go bananas.

5. Multiplying mounts. Used party member on mount and a second appeared. Attempted to simultaneously mount with PC and party member and then a third mount appeared. Attempted to reproduce the glitch again and the game crashed to desktop.

6. PC died but is still standing. He cannot be ressurected. Only solution is to restart from last save.

Other glitches/game issues:
- Loot that lands on ice wall becomes inaccessible, tantalizingly floating in midair.
- PCs upon KO sometimes sink into the ground or float in midair, becoming inaccessible. Disengage and run a good distance from the battle and they will reload closer to you.
- Samson is easy to exploit. Run back up the stairs and defeat him from above. He will not chase you. Easy solution? Magical barrier on stairs.
- Inner cheeks tend to glow/be very bright. We've moved from poor dental hygeine to this? Very minor issue, but distracting.
- When fighting close range to a dragon, PC went flying 50 feet in the air and came crashing down, nearly dying from fall damage. I have no idea how this happened.
- When in lower graphics, hair looks like a solid chunk of shiny plastic. It's so ugly that I traded it for higher graphics and lower framerate.
- It becomes difficult/impossible to find Venatori for the rift mage mission if you choose to do it late in the campaign.
- Why is research loot in the same section as valuables? It should have its own section or be automatically turned in. After DA2's masterful handling of valuables, it seems an illogical evolution.
- Inventory is small, but most maps have no vendor. This creates tedium, especially when you consider the loading screen required for each area and the sheer size of the game. Easy solution? Place a vendor and weapon smith in each town. It's possible to lock them until after the town's chaos is solved, creating a sense of accomplished normalcy. Also, place a vendor in your main camp in each area. Or at least a storage case. This is just more realistic and keeps the game flowing. The main encampment is still crucial, since that's where the war table is and where you talk to your followers, so it's not going to lessen its importance. The Inquisitor should be saving the world, not constantly checking into home base to shop and upgrade weapons. (That's what the little people are there for! To help with menial tasks!)
Apologies for the huge post. I don't know how to make the images smaller. I hope it's entertaining and helpful at least.
Edit
Forgot to add:
- While at the main base, party members either load partially or not at all. Save and reload to solve the issue.
- Like in the second and first game, PCs tend to get stuck on objects, including dead bodies and rocks. This can become really problematic during fights.
- "Seeing Red" mission on Storm Coast shows up way before you can access the area required, causing confusion and therefore frustration amongst players. I personally spent an hour attempting to find a path. Simply adding a "locked door" interaction to the entrance or hiding the mission until it's unlocked will solve the problem.
- When the War Table doesn't react to clicking, use the keyboard, similarly to the conversation bug.
- I'd like to be able to zoom in closer while in regular camera mode, especially since bushes and trees obstruct my vision.
- Hate having to look at the default outfit whenever I'm in Haven/Skyhold. Would prefer if it were whatever was equipped.
- Invisible helmets individually toggled would be a nice addition.
- Lyrics lag far behind songs.
- Finding the 40 "Hard in Hightown" copies in Skyhold is broken. You can simply exit and reenter the hold and click on the exact same copies you've already encountered. The only way to do this quest without accidentally exploiting this glitch would be to stay in Skyhold until you find every copy or perhaps to keep a paper record of which rooms you already checked.
