A friend and I experienced a bug in multiplayer where we all get stuck just after completing a mission, when the camera keeps spinning around the surviving characters. The only way to fix it was for the host to leave. The game would migrate the host and we would then have to redo phase 5 of the mission (last checkpoint I guess).
It happened two times on different maps.

I've seen a few bugs, nothing to jarring to negate from the overall brilliance of this game, and I'm sure all of the bugs will be addressed in future patches, but I'll leave a list of the few I found to be annoying.
1.When modifying armor with a new leg insert (only one I've found glitching is on the mage armor) sometimes the modified item doesn't show up, and leaves your armor looking as it originally did.
2. There are a few cut scenes where your face changes color, sometimes covering the inquisitors face in a dark shade, it looks a bit odd.
3. This bug was already mentioned up top, but during a few cut scenes you get stuck and can't continue. Unless you reload from the previous save, delete it then reload that new save.
These are the only ones I've found, the only suggestion I can add is allowing players to map more abilities to their hot keys, instead of just 1-8 which it currently has. Otherwise wonderful game, I still haven't finished, but I'm very happy with DA:I so far. ![]()

Not sure if this was mentioned before but.. the normal map for Josephine's chair in skyhold.. is reversed, so instead of looking like the light is reflecting off the center of the pillowed back, the parts that are supposed to look "indented" look like they're sticking out farther than the pillows!

Continual game crashes during cut scenes. I know many other people have this problem. Needs to be fixed ASAP.

Not sure if this is a bug or not (perhaps I've just overlooked a feature)...
In multiplayer, it seems you can salvage the equipment being used by your other characters. It doesn't highlight them with a yellow border or flash up a warning. They are just in your list looking like every other bit of collected loot.

Another minor tweak that would be useful to have :-)
- Provide the ability to show certain companions helmets
I tend to switch helmets off, but would prefer for example for Vivienne to always wear a helmet

This is caused by the cutscene FPS fix. You should only run that after creating your character.
It may be caused by this, but it is not only caused by this. I had the same issue, without trying the FPS fix.
Other issues I have had:
(i) Really slow FPS in places. Maybe my setup, but the specs looked like I should be able to have a semi-decent performance.
(ii) Invisible characters in Haven. Cannot interact with them. This is bad.

Fortunately, I found that I had to go into world map then come back to throne room again to solve this problem.

I'd love if Area of Effect Spells had a bit more concise prjection in which the spell will have effect. For example, barrier spell projection does not cover the exact area that the spell will hit, and is often covered/hidden by terrain. It would also be lovely if you could see these projections for melee skills such as Mighty Blow or Whirlwind, as I love playing with Friendly Fire enabled to increase the tactical value of battles, and there's no telling how far will these skills hit and nuke your own party members.
The other big issue I have is the zoom is not nearly as good as it should in Tactical Camera. Often terrain blocks it, and most of the time it's too near and doesn't let you get any decent view of the current situation in the battlefield.

Everything was going great until Hawke arrived :l
My Dwarven female Inquisitor was using the British voice - I customised my Hawke to my standard and when the cutscene loaded, suddenly Inquisitor Cadash was... American. I tried reloading and doing the scene again, but to no avail. My Cadash has apparently had a knock on the head a wee bit too hard and come out with Foreign Accent Syndrome
I also got off my horse at the starting camp in the Hinterlands and flew in a perfectly straight line a few hundred metres south. That was a bit weird.
Might just be my PC, but there are an awful lot of awkward silences during cutscene conversations. Like, really awkward. Probably cause I'm getting bad FPS. Still, it's awkward as heck.
Occasionally, my companions are invisible when at Skyhold/Haven. A quick save and reload fixes this, but it can get quite annoying.
