Effective use of Mighty Blow - Knocked Down?

I'm playing on Nightmare with FF turned on and am wondering how to use Mighty Blow effectively?  Obviously it has to be turned off in tactics or my Inquisitor goes all HULK SMASH on Cassandra.  It's obvious that MB is best used against already knocked down opponents since you get like a 500% damage bonus with both points of MB... but how the heck do you arrange that?  There seem to be few ways to knock down enemies and most of those involve my 2-handed inquisitor using an ability to do it (Mighty Blow, Earthquake).  And enemies stand back up from knockdowns immediately.  So my inquisitor doesn't have time to chain two of his own abilities even if he has the stamina.

So we're left with Cassandra using Shield Bash or that Bull Charge thing, right?  But these guys stand up IMMEDIATELY.  It seems almost impossible to get in a mighty blow while they're prone since there is a bit of a wind up for it.  And that's even taking the risk that Cassandra will be able to get out of the line of fire and not get insta-gibbed by my inquisitor.

Anybody have a better way to do this?  It seems like the only way is to have my 2-handed inquisitor loitering around waiting for a guy to get shield bashed and then instantly, and I mean instantly, swing his Mighty Blow which is quite problematic on Nightmare/FF.

To be honest on FF/Nightmare I'm almost tempted to run two sword and board people?  My guy is really squishy and doesn't seem to be putting out damage like he should given the above constraints with getting MB to go off properly.  Advice appreciated.

Sword and board is a lot safer with FF. I wouldn't mind running two of those. I had to turn FF off for my Two-Hander warrior. Maybe if I could at least see the hitbox area of effect I'd turn it. But probably not since it seems to have 360 properties.

Guess it's a change to make the game more "actiony" but it doesn't mesh well with FF what so ever.

I found one very good combo for FF/Mighty Blow.

As long as you have the stamina for it, using the upgraded "Grappling Chain" ability on the Battlemaster tree followed by an immediate Mighty Blow does very well.  Because grappling chain pulls the enemy next to you and knocks it over, stunning it for 3 seconds with the upgrade.  So now you have a prone, stunned enemy sitting next to you which you can just HULK SMASH with mighty blow.

Hopefully I find another couple of good combos.  Right now the only other one which I find works is freezing an enemy next to Cassandra, having Cassandra Shield Bash it (which I believe gives one combo bonus) and then hitting the now lying down enemy with Mighty Blow for another big hit.  This is much more difficult since it requires perfect timing AND Cassandra can easily get gibbed by my mighty blow.  But it does work.

The upgraded Grappling Chain + Mighty Blow is very easy, it just takes lots of your stamina.  It works every time with no danger to your buddies though as long as you aren't standing next to them when you start it.
