Varric leaves the inquisition if his approval is too low. I'm assuming that it happens to almost everyone. Sera is the hardest to please, so she's likely the one to leave if I don't meta-gaming and do what she likes.

While I am barely into the main story; instead heavily enjoying all the side quests, I do know that losing a character isn't an arbitrary thing.

What I mean is, you can get approval as low as you want, it won't matter, at least for most of them. What does matter are very important, very obvious triggers; i.e.

I hope not because I have a feeling that Cassandra hates me with a passion and she's hilarious.

But then again she can't really leave since she's sort of our boss, right?

Sera is the hardest to please, so she's likely the one to leave if I don't meta-gaming and do what she likes. 

 

Hum I guess my future Elfquisitor will not get along with her since I play my dalish characters as Elves Master Race :P

Lol I'm not the only one then. I swear it is like if I take a breath Cassandra disapproves. Thank the maker I'm not romancing her right now. I would be losing my mind.

I thought Cassandra would disapprove of me more since I'm pro-mage, but I get along with her more than Sera. She disagrees with every of my policy. Not everything about killing the bad guys, Sera.

Varric and I are best buds already. You have to be a real jerk to make him leave...or yeah, as someone else said, let Hawk die (I'm still confused why people keep hiding spoilers on a spoiler forum). But I can't say for sure since I'm nowhere near that quest yet.
