i have encountered this twice in my game, where i suddenly lose the ability to swap between my controlled party members and nothing fixes it other than leaving the game because i was in battle and couldn't control anyone other than my main, the last i remember it specifically happening as i was leaving the ruins on the east road off the crossroads in the hinterlands.

Maybe try powering the XB1 down by holding the power button for 5 secs (or more). This will completely shut it off, then turn in back on. Maybe disable "resume games" in the XB1 system settings, too.
