At first, the problem was just happening on the conversations, but now it happens while I'm playing, and seriously, there's no reason.
I'm using EVGA GTX 285, with driver 185.85 (yea, it's old, but I was using 191.07 and 195.39 before and the problem was there).

It may be a problem with the Nvidia Cards and this game. I am having the same problem on an 8600 gts

I'm having the same issues. I have 2gb RAM, Nvidia 9600 GSO.

I have pretty much the same as Metal Paladin, and the stuff on his screenshot's top, i have it in my game, and it appears on my second monitor aswell, i have updated my driver, trien lowering graphics level and done everything i could think of, but it still happens, 5-30 minutes of playing and it happens

i have the exact same card as the OP. cept i get kind of flickering of the clothes of some charecters and major fps drop in denerim when looking at the market, and sometimes the blood pools render in red squares on the ground.
however once i upgraded my drivers the problem seemed to go away untill i restarted my computer and then they all came back...

intel core 2 at 2.13 ghz
ram 4096 mb system only see's 2986 for some reason

i'm getting artifacts too on my 9800gtx. spec are:
q6600
intel d975xbx2
2x1gb pc8000
9800gtx
win xp sp3, 195.62 drivers 1680x1050
my gpu's temp never exceeds 55C and i have tested it with AtiTool many times for artifacts and there were none.

I'm unsure how reliable you can expect AtiTool to be on an NVidia GPU. How high do you have the graphics set on your 9800?

AtiTool works fine with nvidia cards too for that purpose. I have put my card under tons of benchmarks as well and in all other games it's fine, no artifacts what so ever. settings are:
AA, AF and vsync are application controlled
max. 3 pre-rendered frames
texture filtering is high quality
in game:
1680x1050, detail very high, AA 8x, texture detail high, full screen, vsync, frame-buffer effect

my GPU fan speed i put up using Riva Tuner before playing what were your GPU temps like when it happened?
how do i know it is temp related i always have RivaTuner hardware monitor running to keep an eye on temps, memory/CPU use while gaming it is hot this time of year in Australia but i keep an eye on these things all year round
using 190.62 drivers they work fine so not bothered installing 195.62 for now

9800 GT 1024 MB here, no problem with 191.07 and 195.62, constant 61c even on a hot humid day. But I've seen than on my old 8800 GT that was overheating way above 75C.

sounds like quick-switching errors to me. windows loses focus from one app/game for a microsecond then has problems returning to its previous state. what ever happened to windows dedication to gamers by removing quick switching and focusing on multi-core processing?

This is probably a remote solution but have you checked inside your computer for dust buildup. Dust building up over time would explain why the problem has gotten worse.

I don't have artifacting, but I had a problem similar to the OP when I first ran the game, and disabling antialiasing cured it for me.
