I did it Eternal Glory and Fame is Mine!!! I've found the Location of the Bugged/Hidden 12th Mosaic Piece. Turns out it was actually in the Wolf Den near Redcliffe Farms as stated in the Prima Official Guide, It's just hidden inside of a cliff. Unfortunately, I was unable to reach it I was just able to find the Location. Hopefully knowing the location helps, and if someone is able to access the location please share how you did it. Thanks!
That's crazy. Great find though. (Assuming it is the last piece and not a texture glitch *Highly unlikely*) At least we now know it exists, and it IS bugged. Something BioWare was unwilling to even confirm nor deny.
You can mark it like lootable item using cursor, so it looks like thats it. Now we need to find way to clip/cheat there.

I've gotten to the side where the Mosaic is...but being inside that rock practically makes it impossible to grab.

Thanks!!! I'm glad everyone is enjoying it. I've also been able to get up to the cliff behind the invisible walls and I have tried every method to warp into the cliff with no luck unfortunately as it is encased in solid rock. Trying to use the horse dismount trick just teleports you through the cliff to the other side. So unless someone can invent a NoClip cheat/hack it looks like we'll have to wait for Bioware to officially patch it. Best of Luck to anyone else attempting to get through though.

Not an official statement, but I believe EA is willing to sell you the dlc bonus patch for retrieving bugged items. It will be a combo pack with all seeds, all mosaics show up on walls, bonus exalted plain region, and all mounts all for the very low cost of 39.99 or you may use your already bought multi player platinum currency. Yay EA!

While I'm absolutely thrilled that things like mosaics and seeds were included in the game, I have to say I'm rather disappointed in that it's clear now that no one verified in testing that they were all actually acquirable. (It seems a bit cruel even unintentionally to tantalize completionists with such things! >.<) Not to mention that the issue hasn't been corrected yet, a month after release with two patches already in the rearview mirror.
Please take care of this, devs. These should be tiny fixes for bugs that should have been squashed a long time ago, and the neglect here is becoming an unfortunate detraction from the DAI completionist experience. I understand that a project of this scale is bound to have wrinkles that will take a bit of time to work out; I'd just really like to see these things cleaned up before so much time has passed that they're passed over entirely in favor of future projects. I look forward to those projects with great anticipation, but they can't be excuses for leaving DAI unfinished.

well sorry to break this to you but... just keep in mind that you did your best to complete it and you succeeded for the sake of completing the game 100% :3

Of all of the things in this game to have probably thousands and thousands of people looking for I am sure Bioware didn't think it would be a simple collection item. Maybe they will understand that people are more anal retentive then they think who play these things and have testers come in and check these things before they release a buggy product.
It's one simple item, but it's sloppy considering there's no reason they couldn't have someone go back and say "Hey, go check all of the collection items and make sure THEY AREN'T STUCK IN A WALL BEFORE WE SHIP!!"
It's just a small part of a greater whole. They put it in the game after all.
Maybe Corphy played a trick on us he is still bitter about the falling mountain thing back in haven, he relocate the mosaic piece so we will get frustrated as he did when a mountain fell on him
I have a way where I might be able to go under the map and travel there, but I don't know if I'll be high enough to reach it, I'll give it a shot and let you know how it goes.
Edit: Nevermind. The place where I started is far away from there and there is too much instability underground to make it there. If I fall through the ground somewhere or don't time a jump properly I'll have to start over again.
So I tried to clip through with SparrowFox method and I was catapulted from the texture. I could swear that before the patch I could clip through walls.
