Another post with everything to find in a map (check the collections link).
Those are copies out of the Prima Guide, and do not show everything in all the regions. It only shows 9 of the 10 mosaics that should be in Hinterlands (not counting the two in the dwarven city)
I just did a fresh start and went over to that wolves den in Hinterlands, there is no Mosaic there, I dont believe one was there on my first playthrough either.
I'm also at 10/12. Rechecked a few spots last night to be sure.
Someone's earlier list stated there was a mosaic piece in one the NW dungeons. I don't believe that is entirely accurate. It is resting near a spot next to the astrarium.

Woops, not next to it, but near it. There's a path that goes up a hill. There was a few things to loot up there along with the mosaic piece.

THere is one in the NW dungeon, I just got it today.
This is ridiculous. I've been EVERYWHERE in Hinterlands and I only have 10/12. I've been searching outside the forums for anything on where to get all 12. Reddit and Tumblr haven't helped; just more people sick of looking for 2 pieces that don't exist?
I wonder if BioWare is aware of this and it's development bug? Some joker in programming never put the two in there. We go trolled!!!!

Area in WA is indeed blocked again. Damnit, I recall actually running through the area when the dragon died because so many pillars collapsed. Obviously I missed that entrance completely..
Sigh....just came over the same thing. Wonder if it will get fixed. My OCD is screaming at me to reload but been soooo long since i killed the AHD that i don't REALLY want to do it all over again

I have a case of the 10/12 on 2 of the mosaics the one in the dales and hinterlands I have 10 of 12 and 4 for free our slaves

Thanks for the confirmation. I must have grabbed it already unless there's some secret area in that dungeon that I'm not aware of.
I found 11 mosaics in hinterlands, though I believe that one of them was for freed are salves, as there are 11 on the prima guide, but I only have 10/12 for the fall.

The second map shows one piece in the wolf den but I still have a doubt there is one there. I have checked all of these 11 locations (including the couple in Vallamar) and still have 10/12. So either the second map is incorrect or there is a bug that didn't register one of our pieces.
Thanks to this thread, I've got 12/12 in the Western Approach and 10/12 in the Hinterlands.
Checked out Maxulic's bugged ladder. Verified, although I don't think it goes anywhere. I used tactical camera to look at the other ladder in that upper area and it's impeded by a rock at it's base. It's possible this area is damaged in the fight with the dragon, which makes it inaccessible... I doubt that too though.
Checked the Wolf's Den, found nothing. Also couldn't find any youtube video of finding one in the Wolf's Den - can anyone verify that one?
Perhaps the other two are found later in the game, or in another area. I've scoured the Hinterlands pretty well.
There are 2 ladders in the dock area (that was actually the first one I managed to spot since the bugged one sometimes doesn't show when searching). I am pretty sure that the first one that is bugged is supposed to allow you to climb at the level of the second ladder which will finally allow to reach something... a mosaic piece hopefully.
My guess is that you are supposed to follow the arrows but the first ladder is not only invisible but misplaced (why would we climb inside the mountain?) so we are missing the first step.
There is a flat area represented by the blue line, I tried for 1/2 hour to find an alternate way to get to that level (even managed to reach the high ground on the right where the dragon lands at some point in the fight thanks to a companion that ended up there) but there are invisible walls everywhere so it is pretty obvious that the only way to climb it is through these wooden structures.
So to recap, this is the situation to date as I understand it:
--No one has found more than 10 of the 12 pieces of The Fall, which are all supposed to be in the Hinterlands.
--No one can confirm that there is or ever was a piece in Wolf Hollow, which is marked as a mosaic piece location by the Prima guide.
--No one knows for a fact where to find the last piece or two (depending on whether the Wolf Hollow one is supposed to be there but is bugged or if the guide is just wrong). One or both may be hiding up those bugged ladders, which would be like the two pieces of the Archdemon found in the dragon's lair in the Western Approach, but it cannot be confirmed.
At this point, unfortunately, I think we're stuck praying for devine intervention. (Perhaps that horrible pun will inspire some? 
)
Here's what I do know from my experience. I do know there is at least 8 confirmed from the Hinterlands (excluding the Dwarven Outpost Dungeon). There's 2 in the Dungeion. I believe, but have yet to confirm, the other pieces may be in the Storm Coast and/or the Fallow Mire. The guide claims each place has one mosaic piece.
Not all the symbols on the prima map are accurately placed.
Also if you look very carefully you can count ten mosaics in the hinterlands map of the prima guide. There is one hidden below a glyph and another mosaic symbol. I think it is a misprint since i never got closer then 10/12(including the 2 from Valammar)
Also in the prima guide there shouldn't be any mosaic pieces in storm coast and the mosaic piece in the Fallow Mire is part of Freed are slaves.
Same here, 10 of 12. My OCD is making this a real suffering, since I just don't feel like doing anything else till I find that damn mosaic.
I can confim that there is nothing up the ladders in the dragon valley as far as I can see.
I got up there by using the "f" key and some camera angling to get the ladder in sight after that there is another ladder up there. I got to the next platform with the fade step skill.
But yeah in the same boat as you guys would be so nice if we could get some word on the matter if its just bugged or if the pieces are outsite of the hinterlands.
I hate this stuff as annoying as the 20/21 regions in exalted plains problem

awww, just made this account to tell people that i didn´t find anything after making it up there.
This may be inaccurate since it's not personal experience, but something I saw someone else mention in a post in another topic (that I cannot find, sorry) - supposedly you'll need the Deft Hands perk to get all the mosaic pieces, as some are locked away behind master locks (I assume in one or more of the locked houses, as those have been the only master locks I've found in Hinterlands; iirc one is at Dennet's farms and two others in Redcliffe Town).
This may be inaccurate since it's not personal experience, but something I saw someone else mention in a post in another topic (that I cannot find, sorry) - supposedly you'll need the Deft Hands perk to get all the mosaic pieces, as some are locked away behind master locks (I assume in one or more of the locked houses, as those have been the only master locks I've found in Hinterlands; iirc one is at Dennet's farms and two others in Redcliffe Town).

So I do see something underneath the Dead Ram Gorge mosaic piece (in the NW corner under the glyph), and it may be a mosaic piece, but in other places where two pieces overlap like that (e.g. the dragon lair in the Western Approach) the pieces are both found in the same location or feature (that is, they should both be in the same cave or what have you). Yet so far no one seems to have found two pieces in DRG, just the one, so you're likely right that it's a misprint (or meant to be there but never actually shows up).
If the marker in Wolf Hollow is simply inaccurately placed, then it must be terribly inaccurate indeed for no one to be able to confirm the piece's existence. I know the one by landmark 17 is a bit off (the cabin is ESE of that landmark, not directly on top of it), but not by too much. When I get back to my PS4 I'll poke around the other side of Wolf Hollow (that is, approaching from the east) to see if it's possibly back there somewhere, and I'll have another look around the outside of DRG as well to see if there's a second piece hiding in the bushes or something.

To clarify a bit: The perk is necessary for a piece of The Archdemon in the Still Ruins dungeon in the Western Approach. (It may also be required for pieces of the other mosaics, which I haven't yet finished.) The places you list in the Hinterlands are all masterwork-locked, but none contains a piece of The Fall. However, there are definitely locked cabins in the Hinterlands that require a rogue (but not the perk) to open in order to access mosaic pieces. The one in Redcliffe Farms to the east of the masterwork-locked one comes to mind, for example.
