The most annoying part is the fact that even if you get all Mosaics of a certain set, they'll still show as incomplete in Skyhold. After I maxed out The Archdemon, for example, only 11 Mosaics show up and the picture is incomplete. ![]()

I'm certain there was at least one or even two in redcliffe dungeons when you're sent to the future if you choose the Mage mission (and most likely the same are available if you do the Templar mission instead)... can't confirm whether they're both from The Fall though. If you miss these, you don't get a second chance.
I'm actually about to do the mage quest, so I'll check.

I have even more problematic... problem. I missed this cave after the fight with Abyssal Dragon and... blocked it while doing the Dark Spawn quest which is also bigged because I can't find Servis after his "death". Walking through walls cheat would be good or oportunity to reset the Dark Spawn quest. :/

Edit: Just completed the wolf den and Astrium caves, found no mosaic pieces in either. I also searched around the area marked on the map in the players guide near the wolf den with no success.

By the way, there are actually 10 pieces shown in the main Hinterlands collection map of the Prima Guide. It's really hard to see, but the "missing" one is underneath the NW icon. If you look very closely, you can see the right edge and bottom-left corner sticking out between the glyph and other mosaic icon. Makes me think that the bottom one is supposed to be the one that everyone's finding in the cave, since it's closer to the glyph.
That said, I couldn't find the piece even after scouring the area.

I didn't spot that! Nice eye, that means every piece besides the 2 in the taig (forgot the name) are visible on the map. I checked in and around the NW dungeon, no success.
I think this means that the one in the wolf den, and one of the ones in the dungeon are bugged. Untill a dev says otherwise, I think this should be the agreed assumption.

I am at 10/12 in Hinterlands. I thought it was 11. Baffled.
From the list earlier in the thread, ,bold to confirm which ones I picked up on my clearup run. All locations *appear* to be empty (wish I could stop the search dinging on plants/ladders/doors/braziers):
Fort Connor - need clarification. Appears empty. Is it in the landmark location?
Shack near Fort Connor - need clarification. Both obvious locations are empty.
Shack east of Tyrdda's Lover (this is a locked door.)
NW Dungeon - empty.
fort @ redcliffe road - need clarification as I'm not sure where it means. There's a gatehouse and more of a proper ruined fort closer to Redcliffe.
Forest Villa - SW tower (this is the one with three ladders)
upper lake cabin - empty.
Winterwatch tower NE - empty, but clarification could be useful. Is this the tower room with the kid making the potion for his mother or the one with the artifact?
vallmar - 2 - went back and double checked this area. Empty, one certainly collected before hand from end of a bed.
I'm not able to screenshot to help out sadly.

I think its fairly safe to say that the listed ones are there...but we still have 2 missing ones. I've been round three times to double check and they're all empty of pretty much everything now.
Theory: they're in the time travel section/Redcliffe Castle, which seems dumb (and there wasn't a whole lot of it to be honest, I was all 'omg, this is..neat?' and looting everywhere),
or, they're in a cave location and we've not investigated, as in most other regions pieces are inside caves.
Or someone can't count and screwed it all up.
Hopefully the patch they're working on for Dorians quest triggers will fix a few other things too.

My though is that this is a bug and it will be fixed, but since I have not yet received a reply from a dev. to this it is only a presumption.

Just wondering what the point of the Mosaic Pieces are. Do they make a picture in Skyhold if you find all of a set?

Yeah. You can find them near in the entry hall, near Varric, and near Vivienne's higher balcony.

To sum things up, the mosaic pieces we all seem to be missing are the second in the NW corner and the Wolf Hollow/Astrarium Cave. A few people seem to have found the Wolf Hollow one, though there have been no screenshots. Other people have said that the Wolf Hollow is actually in the Astrarium Cave and is a Freed Are Slaves piece instead. Someone said something earlier about there being a hill near the NW Astrarium with a piece, but that it wasn't the glyph cave piece, which I'm a little skeptical about. All other 10 pieces are accounted for my the majority of posters.
If anyone on this thread finds a piece in the Wolf Hollow/Astrarium Cave or a second piece in the NW corner (not in the glyph cave), posting a screenshot would be incredibly helpful.
As a spontaneous thought, has anyone tried to go to Wolf Hollow before starting the Safeguards Against Looters quest? The box for that quest might overlap or remove the mosaic piece. I'm too far along in my current play-through to check that myself for a while, though breaking the box should still be possible. Potentially, the mosaic piece could be hiding behind it and won't ping on the search radar.

No, I'm relatively certain there are no pieces in the Wolf Den OR the "NW corner". It's a matter of not putting these missing pieces in the game to begin with, imho.

Another post with everything to find in a map (check the collections link).
Nothing new, it shows 9 pieces including one in the wolf den which still has not been verified. Till someone posts a video of the mosaic piece, there is none in this location. So we are back to 8 pieces in the Hinterlands and 2 in Valammar, which makes 10/12.

I got both of those pieces. The "NW corner" one is in the cave where the glyph is, right? Definitely got that one. And the one in the Wolf Den was definitely right where savvyraven said it was. However, I'm still at 10/12 even with these two, and I did get all the other ones on the OP's list.

Just started a new game and will see if I can screencap the location of every Mosaic I find, alongside a map screenshot. THIS WILL BE SOLVED.

Either some of these locations have more than one mosaic tile, or even Prima didn't know where all of them are. (It is possible some don't appear until later, as their guide to Caprice Coins and Secrets at the Winter Palace doesn't mention some don't appear until the final act, when the Royal Apartments finally open).

I'll make a new playthrough to test this either tonight or tomorrow. 2H warrior so I can look in the cave before any other quests activate, and to see if I can smash the box afterwards.

As I previously mentioned, there are 10 pieces shown on that map in the main Hinterlands area, two of which are in the NW corner, by the glyph cave. It's very hard to see (especially on a screen as opposed to in person as that is, in fact, the map in the Prima guide), but underneath the easily visible mosaic icon and glyph icon, there is actually another mosaic icon, slightly up and to the left of the first tile. The piece in the cave with the glyph is the one everyone is finding and seems to be the icon that most people miss, since it is closer to the glyph. The top mosaic icon is the one that most people can't find, which may or may not be bugged and may or may not be in the same cave.

I agree with you completely. I was saying that according the map and what people are saying, those two are the ones we're not finding and/or are missing from the game or bugged.
