Unfortunately, I was unable to destroy the box from the safeguards against looters quest, have not tried checking the cave before hand though, I'll try that at dune point.

I found a mosaic piece for "The Fall" in the NW corner of the Hinterlands as indicated on the Prima map. It's definitely bugged. It was tricky to get to, but fortunately I took screenshots. I have added instructions on how to get there to the screenshots. It gave me 11/12 mosaic pieces for "The Fall".

Mess as it is, I've (un)fortunately confirmed this is the 11th piece of the mosaic with this method. Definitely looks like a bug btw.

I hope you meant the placement of the mosaic piece, not my description.    I tried to make it as clear as possible. Hopefully it's fairly easy to follow, even if it's definitely bugged.

 

Now if only someone can find the last piece. Hopefully there's a way to get to it and it's not just bugged worse than this one is.

You definitely made clear instructions, but yeah I did mean a mess on bioware's part, LoL. Hopefully someone turns up the 12th piece eventually, but with what you just proved it could be anywhere now...

I just managed to get up to the area around the top of the Wolf Den cave and completely circled it (invisible walls and all). There is no sign of a mosaic piece up there.

I have also confirmed the second mosaic piece in the NW corner. I definitely believe it is bugged, and I think I know why:
Given that it is just a tiny distance from the "roof" of the dungeon (it's a bug that you can even access this area) and given that when you stand on the dungeon's roof and use the search function, things INSIDE the dungeon are highlighted, I believe that whatever causes the mosaic pieces to spawn, caused this one to spawn just enough away from where it's supposed to be, that it would have spawned it in a wall, so it changed the vertical position to still spawn the piece in open air.
If my theory is true, we should search the tops of the cliffs surrounding wolf hollow as much as possible, perhaps the same thing happened with that piece as well.

Something that may help our search, dismounting a mount (I used the red heart and iron bull) into an invisible wall allows you to pass through that wall. If they patch that and not the mosaics, I'll return my copy of inquisition to game stop.

I did manage to get up onto the clifftops around Wolf Hollow (as I mentioned in post #132), however more eyes up there would be appreciated. I will get back there shortly and send a series of screenshots about how I got up there so others can take a look as well.

You know I spent an hour trying to get up there last night to have a look... guess I had the right idea lol.

I will stress "tried" because my lack of gaming skills / jumping apparently stop me /blush.

 

Back to trying to get up there!

Just got on top of wolf hollow and I've searched the immediate area with no luck. I've been able to bypass the invisible walls so far, but searching is taking it's toll on my patience. Will continue searching towards the valley of the burned man in a few hours.

The way I did it was to go up the incline in the NW corner of the cave to where the hexagonal pillar rock formations are. With some tricky jumping, you can climb up those to a small grassy patch surrounded by invisible walls to the north and west. You can carefully make your way east on to the ledge that covers the incline you first walked up, and from there explore the surrounding area above the cave.

Don't worry. It took using my mount's jumping to get me up there. I didn't try ChadaMonkey's dismount to get through the invisible walls, though. So it's definitely worth another look up there.

Even jumped down the hole a little further on and nothing... lucky switching to another character makes the other one unstuck lol

ok got up top had to change mounts, the CHarger and the Nuggalopes were to fat to make it but the dracolisk worked fine (first attempt with it). time to explore and spam the hell out of L3.

While up there, there was nothing better to do. I was missing Cassandra and my nice soft bed in Skyhold.

I slipped and ended below the stone spiral (roughly the location marked on maps) in the den but nothing there.
