I just got to skyhold, and have left/come back a few times.
Cassandra has a symbol on my map, and has shown up during conversations with other characters. However, I cannot locate her to talk to her, nor have I been able to since I got to skyhold. Is she hiding, or is she possibly just bugged out?

I'd call you something along the lines of being dumb but there's no reason to do so because most everyone else knows this. The simple fact is, in the map her icon is at the training dummies. The main issue is she's not there at all where she should be. This happened after I talked to Varric and then the Champion of Kirkwall. She disappeared. She's not even at the war room table. I've even idled there for a half hour and she's not shown up at all where she should be. Its rather annoying that a npc you want to talk to is just poof begone.
That kept happening to me. How I managed to resolve it was save just in front of where she should be, shut the game, start the game again and load the save.
