I have been playing through the DLC and I decided I would collect the mosaic pieces I was missing. I was also 20/21 in EP. Today, when I fast travelled from Western Approach to Dalish camp in EP, the regions found updated and I got 21/21. I hope it works for you guys! And yes, I had already fully explored EP and done everything there. Weird!
