Sometimes I see a red skull like icon on creates in the world, and these creatures seem to be harder. What exactly does it mean? Are they elite, or higher level?

It means those creatures are high enough above your current level that they could, conceivably, kill you and your party with relative ease.

 

I found that out the, um, hard way.

I saw that that Hinterlands. I was a level 6 mage fighting the demons then bam, got 2 shot by some weird looking blue fireball. It wasnt untill I saw on reddit that the red skull = monster is to high level.
