Am I the only one that seems to have accuracy problems with this skill? I have to cast it 5-6+ times before it works (Does not use stamina on a "fail").

 

I press it, I see me throw the knife at the target, no damage, repeat ad nauseam until it finally does damage.

You could be missing them - I've had it happen a couple of times, as well.

 

When they're further away, they probably move out of its way. Stealth (gives crit bonus, anyway) and get as close as possible. That works for me and make sure there are no obstructions.

 

I've missed with the attack, but I haven't had any glitches or bugs on it.

Best thing I have started doing is I have my assassin rogue with dual daggers that each have a 10% chance to activate hidden blades, it works really well.
