Hello fellow Inquisitors!!!! So after reading a lot of feedback on the game's Facebook page and on this forum, I decided to do a comprehensive list of quality of life improvements...
Well done OP, this is the most constructive improvements thread I have seen.
I think DAI is a terrific game, but items here add great value even if only as a template for DA4.

(One aspect of the female PCs isn’t as much a matter of taste—the hunched, bow-legged cutscene body is very different from how the rest of the animation for them.)
thats because most body animations aren't unique to this game, those are old reused animations from previous dragon age and mass effect games.
They couldve made new ones ofcourse to make it feel fresh and well, better, but resources were spent on other things, like dracolisks, drapes for skyhold and multiplayer.
Edit2: while i agree with mostly every comment here i find it rather insulting to the developers that people still demand to change the looks of female companions. Most comments are objective, but changing companions is subjective as some people like them and some dont (which actually the forums prove, there are a lot of men liking romanceable female companions) Changing companions' looks based on someone's personal standards of beauty is wrong. OP, its your call of course, but i don not think it should be in the main wish-list. Much like you did not include comments regarding reduction of open world exploring, since those were purely subjective. There were enough topics insulting female companions. This topic is great and has really good improvement ideas. Subjective points only diminish it.
Honestly I totally agree with you, and I put it there because nobody asked me otherwise. I was honestly hoping someone would tell me this (finally thank you) because I have spent alot of time discussing this already on other forums, and I usually end up the only one saying they look okay against an angry crowd of internet haters. So, that's why I put it there but now i'll remove it.
Sadly most people don't understand it all comes down to opinion and that it's a matter of personal taste since it doesn't NOT affect gameplay in any way but hey, welcome to the internet!

1. Monster scaling and drop rarity scaling. The problem with Dragon Age Inquisition is that so many side quests exist that if you unknowingly do to many of them you can out level the actual story. As such even on nightmare you can demolish the game with ease since the monsters damage and hp and resists and so on do not improve if you happen to level higher than the content itself. As such monsters need to be scaled up. If you are level 20 and you go back to finish side quests you have not done, there is no incentive or reward for doing so besides the desire to try and 100% the game since there are no side quests for being that strong.
2. Make crafting materials worth twice as much as the previous tier items so that one can sell materials they farm in order to buy other things they need. Some items from vendors cost as high as 15k gold which is quite expensive when most materials sell for 20 gold or less and weapons only netting you average 100 gold and being limited to 75 - 90 inventory space which easily gets filled up with monster research and valuables.
3. A bank , vault, or stash area that can be an upgrade at the Skyhold that can be created in order to store things of value you may not want to sell. Completed runes, upgrades for your armors and weapons, weapons or armors you may receive for a companion you have not received yet, and so on. Doing gather resources war table missions and actually going out and farming resources makes it quite easy to build up a stash of tier 1-3 materials and sometimes it would be best if you didn't have to go through such a large page of materials in your inventory when upgrading or crafting things. Place the vault in this spot in picture, I mean its literally perfect.

4. A way to check overall completion of the game such as you can find in many games like assassins creed for example. Solas' room at Skyhold for example shows overall completion of the main story in a non-numerical value, but there are a lot of side quests as well.
5. Armor appearance altering (like transmogrification in Diablo 3: Reaper of Souls) A lot of items look good, but unfortunately when you craft high item level gear you unfortunately end up with a character with multicolored gear that does not match. If I Have a red helm I would like the rest of my armor to be red as well. If I am using a 2H Sword I don't like very much I would like to change it's appearance to a 2h Axe for example.
6. Taunts don't appear to work on Dragons.
7. Companion pathing is messed up when fighting some things such as dragons. They will sometime run around something they are fighting and then attack. Likewise companions do not attack until you attack something or get attacked. If I run into a group of enemies they should have already initiated attacking already. A mage and archer for example should be attacking them a few seconds before I even get to them myself as a rogue or warrior.
8. Necromancer "Spirit Mark" AI's are useless and do not actually contribute to fights in any real way.
9. Giving the inquisitor access to all specializations not just the ones similar to the base classes of others. The inquisitor is special and enabling this would create far greater build diversity and party creations. What if i want to be a rogue that uses 1h dagger and a shield and shield wall so that i can back stab when possible but protect myself in unavoidable damage moments.
10. Requisitions do not matter, they steal your resources, they don't have an impact on the story or others views of you. The power is negligable there are plenty of side quests. I am 100 hours in and sitting at over 150 power.
11. Make ping show loot through walls and solid objects. Indirect line of sight makes it so items do not show up when you ping.
Lot of the stuff I wanted is already on the list, but there is one that I would just love to have in the game:
Make the inquisition army customizable! Just stuff like colour would be enough. I mean really? The mightiest army in thedas is dressed in oh-so-intimating combination of light colours of green and yellow. I want my red/black stormtroopers!
PS: Mod support cannot be overstated.

Power in general is useless pretty early in the game. If you do sidequests and exploration, you quickly have more power than is ever required. Seeing as this game was largely sold on the size and scale of the world, its odd that players who do aggressively sidequest are penalized by having underleveled gear, and a surplus of a resource which has very little use. Let Power be spent on more Inquisition perks, more character skills, trade it for gold, be able to do more than 3 Operations at a time, ****** something.
I would also like to see them disable the hand animations in all cutscenes. The folded arms are OK, but the gesturing is terrible. Its absolutely appalling. The hands are also badly rendered. Just have the characters fold their arms or keep them still. Its nice that they tried to make the characters more "real" by having them gesture, but its so badly done it makes them even more robotic and inhumane.
Along with your "more hairstyles" improvement should definitely be an option to recustomise your character in game, much like the black emporium mirror in DA2.
More variety of colors in designs when creating items or item pieces (grips, hilts, etc). Some armors have every piece change color (some light armor for Solas recently), and others are a static appearance regardless of what you choose (some dagger has a white hilt with a blue guard and pommel regardless of choice).
Please add a Compare button when making armor, so we know if better than current (been making notes on paper due to this).
Make the search ping clearer, either show the item on the mini-map, or highlight it with some kind of beacon, like the blue waypoint marker on the world map for example. Some places such as the Exalted Marches, makes it incredibly hard to see the items, even when highlighted.
ALSO, add the option to change the color of the outline. Personally, a light blue would be good, even stark white (prefer the blue), as I've yet to see anything that would cover up the blue. Maybe just add that as part of the options in the same menu as the helmet (interface menus I think it is).
A way to adjust length of hair in character creation.
Pluss add Blackwalls hair. = D
Have a way to change Haircut, tattoos, makeup and maybe even voice.
In regards to voice, be able to adjust the Pitch. That alone would make a huge difference in my opinion. Please! I dunno what the Qunarri sounds like yet, but would appreciate the ability to make certain voices more believable if there isn't enough variety in the first place.
I think they needed more scenes for the romance. I love romancing characters in the game but hated having to do important quests in order to get anything special.

* Rewards Loot must scale with the character. Auto Loot/ Loot all etc
I found almost every boss i encountered apart from the kingpin highland dragon in the pit, dropped subpar loot, for the challenge involved in defeating them. Also many bosses that were scaled because of the mainquest line dropped loot that was not relevant to my class & or level. I don't have anything against crafting,but there is something off with the purple loot.
* Wartable / Power & Influence. Ultimately its the same thing.
The same system was in Mass effect 3. There wasn't even an attempt to make it more transparent. déjà vu. Just link them together and be done with it. Give better rewards for sending advisers on missions. An example would be Leliana being sent out for 4 hrs and returning with an underleveled staff. On another note please add level recommendations for the different areas like the main quest.
* Camps Remove those infernal quests that steal your hard earned resources.
The irritating part is i didn't realize i was robbed until the 4th time it happened. I assumed the resource quests in camps were necessary to progress your power enough for the main campaign alas it is not.
*Skyhold Needs more options for the players attire. More renovations on the hold.
An observation on Skyhold itself, I found the lack of heavy artillery on the battlements strange. Or the half destroyed prison with 1 guard potentially housing some of the most deadly criminals in Thedas. Cullen spends all that time mulling over the defenses of Skyhold. To me it seems a massive oversight.
On clothing, If i wanted to wear a beige uniform i would transport myself back to the 70's. Perhaps the purpose of it is to make us hate it so much, when they add DLC for relaxed attire in Skyhold that we are desperate enough to buy. A kiosk in your room with the option to change your clothing would be welcome.
* Guide Game is big ...
DAI needs some casual hints for sightseeing, & locations that are relevant for your level & progression in the main campaign. Hinterlands was such a stunning area filled with quests i didn't realize i was surpossed to move on and come back at a later time. A solution would be to add party banter gently encouraging the player to explore other areas.
* Optimization For PC
2 weeks ago i purchased a 970gtx an upgrade from my 670gtx. A 970 should be more then powerful enough to run a modern game on ultra without framedrops at 1080p. I am shocked lately by the poor optimization on the PC. Not just with DAI but with many other games that have been released in the past 4 months. The latest patch from Nvidia actually dropped my framerate instead of increasing it. In the marketing for DAI 4k was mentioned several times. Totalbiscuit runs 2 980's and cant get a solid 60fps at 1080p. What kind of NASA supercomputer do you think is needed to actually get 4k Bioware?

I'd like to see a system similar to Star Wars: The Old Republic, where you can display helmets indvidually. Not just for the entire party.

More Story, and less trying to be like Skyrim.
It would have been nice to not have the qunari down-scaled to elven size (!!!) to fit the animations of romance scenes. Femqunaris should get their own animations so that they don't have to shrink and look ridiculous and scary.
(I only know this happens with femqunari and Blackwall so far)

I'm going to keep it short, because everything in this game is so polished and glorious but somehow, these obvious mistakes got through QA.
1) The tactical camera. It is really weird to move around and unresponsive. Especially in dungeons. The camera angle is all wrong. Fix this.
2) Issuing orders.
First, don't have AI forget the orders I gave them when someting happens to them or use an ability. For example, if I tell them to hold position, and a dragon reels them in with their wing attack, the first thing that should happen is the AI should move to the set position for holding again. Not staying in their place. Also, if they are to hold position, the AI should be smart enough (see 3) to not use close range abilities unless enemies actually close on them. Instead of running in and staying there. Or they should be smart enough to run in, use the ability and get back out.
Second, I want to be able to give consecutive orders. E.g.: Go there and hold position, then use winter's grasp on that enemy, then use fire wall on that enemy. This would allow for VERY much less micromanaging. You could just plan out each battle in advance.
3) The AI. There's two options here.
Either make them very much smarter about when to use what ability, or let us choose the situations in which they use the ability. The meagre follow/defend options is an insult to previous games.
Or (and this is my personal preference) bring back the tactical slots we had in DAO and II where we could choose which ability does what.
These are the things that need to be fixed. Don't get me wrong, I'm loving this game to death, it is fantastic and I AM actually enjoying the combat. I'm on board with the persistent damage and the 8-ability limit and all other changes you made, even the not attribute thing. I get it. It could just be so much better if these faults were ironed out.
You say you are a company which listens to your players. You say you want damn-near perfection. Well prove it. I've seen the topics on these forums. People are very generous offering your due credit where it's valid. Now prove you are big enough to adress the mistakes.

how about armor upgrades for the dragon armor you get from the dlc just about every other armor in the game has the option of putting better arms guards and shin plates on with more stats applied to them but not with the dragon armor i would like to see that in the game
be able to use war table from inquisition camps or at the very least from keeps
More schematics could be great, been searching everywhere for the schematic for The Whale, alas to no avail.
Overall a greater variety in weapons could be good too.
I think that Main story missions should be Level scaled, as I ran around and was level 20 when I attacked the Warden fortress.
We should've been able to outfit Hawke, afterall my hawke used two hand weapons, while a minor complaint it would've been a great simple option.
Also, Upgradable Armor of the Dragon would be fantastic.
Anyone here notice in a talking scene where one of the characters will ever so slowly sink through the floor? Had that happen a couple of times.
