Most of the upgrade system is pretty easy to understand, and I thought I had it down. Then, I tried to create handles or grips or whatever for some daggers on my sister's character. Despite being located under the Dual Weapons section in the Create Weapon menu, the game wouldn't allow me to add them onto the daggers I had just created, despite the game saying that the daggers I'd just made had an open grip slot. Later on, I realized that the item level of the grips was higher than the item level of the daggers I was trying to put them on, and figured that I'd solved the problem.

Nope.

Trying to put the same grips onto a dagger with an Item Level higher than the grips still didn't work.

What, exactly, are the rules that determine what upgrade items can be added to any given weapon or piece of armor?

Wow. You know the upgrade system is really bad when absolutely no one can figure out the rules that govern it.

I love customization and the game is great but they could have organized and explained how the armor and weapon creation works a lot better then they did.

Creation is fine; that's easy. It's understanding why I can't put Dagger grips on Daggers that is the problem. Or take parts from a unique stave and put them onto another stave. There needs to be a much more in-depth explanation of the rules behind the system, because neither the game or the official BioWare-approved guide explains the intricacies at all.

I've had no issues with it.  I can remove upgrades, except for runes obviously, and put them in the same type of weapon.  That includes the purple unique weapons, I've used their upgrades on my own crafted items before.

I have noticed there are two types of daggers.  There's the "Dagger" and then there's the "Dual-Blade Dagger".  I don't play a Rogue so I'm not sure what the difference is but since I've seen it called out specifically in schematics I would assume they're not interchangeable.

I tried to pull an upgrade from a unique staff and add it to another staff, but the upgrade didn't show up for the new staff in the list of upgrades. Not only that, but I took a look at my sister's character's gear, and no weapons specify "Dagger Grip" vs. "Dual-Blade Grip" in the list of what upgrades they have available. And where do Blades or Knives fit? Do they need Blade Grips or Knife Grips? And what about the fact that I can't put the Qunari Slasher Grip from the Sweetish Fingers into a Raider Axe? The problems aren't just with daggers; I've run into issues with 3 of the 5 weapon types, which implies that the Bows and Two-Handed Weapons have similar issues.

The fact is, the system has far more rules than we are being told, but no information on what those rules actually are. The system isn't just bad; it's a complete train wreck. We need a hell of a lot more information than what we're currently given in order to make the system work effectively. It's annoying that game companies assume that everyone playing their games is all brain-dead and couldn't think their way out of a wet paper bag with a gaping hole in it and the mouth open.

There are two types of daggers.. dual-blades and the single-blades. You probably had a mismatch of grips...
