I cant find it anywhere, however there is a door in the keep to the north in the keep across the bridge which has no funcion but looks as if it leads outside somewhere?

 

It has an interaction notice to "open" it, but nothing happens when i try.

I am missing one region as well.

yep same hapened with me, can open the door, spent an hour running around trying to climb over mountains. Just gave up and continued main story

There is an operation that opens the way IRC.

You have to claim an area near the locked region to unlock the operation. It's sort've near the building the can only be opened if you have the Deft Hands perks.

There are only 2 operations. Did it all but i am still missing 1 region.

Set the locate regions quest as the active quest and it will show you what you haven't seen yet.

I'm also stuck, trying to get to the upper part of Exalted Plains for a couple rifts and Unfinished Business.

 

I did the war room operation that unlocks Ghilan'nain's Grove, but I'm not finding another one to get me to that upper part of the map. Where is this one the map? Somebody mentioned I need to claim something somewhere? Any help would be appreciated, thanks folks

 

(edit: Turns out by the Fens Camp in Ghilan'nain's Grove, there's a path that takes you to the Northern part of the map. Right above the path that takes you to The Dead Hand area.)

The door is bugged because of the bots standing in the path. The messed up part is that that door is the one that opens up into the bell tower and leads up top to where you ring the bell or something to have total victory over this part of the quest. EA needs to move the stupid bots blocking the door. Morons didn't test the game good enough. I did manage to get my horse to jump around the right side bank till I found a man-made waterfall and jumped my horse up the shoulder till I got over the top of the waterfall and get around behind it all and climb my horse up the mountain till I was near the top of the tower, that's as close as you can get tho.

I've read a few posts of this happening in Crestwood as well, where players were getting 18\19 regions. I'm not sure if this is because of the same reason there's an area missing in the Exalted Plains, but I managed to get the 19th region in Crestwood through a companion quest. I picked up a quest from Sera which lead to a small part of Crestwood, in the far North-East of the map. This was the 19th region, and I got it before I even unlocked Crestwood at the War Table. Once in Crestwood, I tried going back to that same area, but you can't, as it's entirely ringed in with rocks.

 

So I'm thinking\guessing\hoping this is the same reason I haven't gotten all the regions in the Exalted Plains. Due to my completionist nature, this is really bugging me.

 

If this is indeed due to a quest of some sorts & someone happens upon it, please post here so I know who to talk to or where to go to pick it up.

I've been the same, I run for everplace, and i dont'. get 21/21, i'm playng in a PS4, a friend of my, play on PC and he get 21/21, maybe i'ts a console version problem?
