For example, when I go to the blacksmith in Haven, Varric's crossbow (Bianca) isn't in the list. But I have upgrades for it.

 

How do I apply them?

Select the table that modify weapons. Your main will be selected but you can select the others by clicking the arrow  keys above your character. Select Varric and his crossbow will be available to upgrade
