Depends on platform. I've heard scars and freckles are unavailable on 360. Not sure about PS3.
After the last patch, they are not visible on pc either. I picked a random one for my qunari. Can't wait for the new patch to see how Cassandra's interrogation has affected her...

I think that depends on the graphics settings. Like on medium or lower the scars don't show up. I haven't tested it myself (playing ultra), but that's what I've heard. Try increasing the texture settings or something and the scars may appear.
