"Here's a winner: go into your journal and hover over everything, everywhere, until it no longer has the ticker next to it that designates that it's new. Of tremendous importance here is that many times completed quests have to be hovered over in their section as well. Likely a bug, and it probably causes interference in the system, messing around with other stuff in the process. Now do the same with your inventory and codex... yes, it's annoying. But it seems to clean things up for many players. You might try doing all this while disconnected from all account settings too; in my experience that has helped greatly. Once you're done, save your game, exit to the main menu, and then jump back in. "
This solution worked wonders for me.
Almost immediately after clearing all the little stars from my codex and all areas of the journal, I started hearing party banter. I'm at about 50 hours of gameplay now and I've just started hearing Cassandra and Varric banter for the first time, and I've had them both in my party for most of those 50 hours.
Exiting to main menu and jumping back in wasn't necessary, I just cleared the unread marks and it worked. Annoyingly however within 10 minutes I had a whole gammut of new unread stuff in the journal and codex just from wandering around and questing. Really needs a "mark all as seen" button.

Just to add to this, it doesn't seem to have fixed the problem completely; just kind of jumpstarts music and dialogue for a while but it seems to glitch itself again within an hour, either because of new unread content in the journal/codex (especially the "completed" section of the journal.) or just because it felt like it. At that point switching the audio settings to TV, exiting the menu and saving settings, waiting a bit, and then resetting it back to whatever (headphones in my case), seems to kick it back into working again for a little while (also clearing the codex/journal again if there are new entries when it stops working.)
Absolutely tedious ad nauseam though.
