I had a weird glitch where i fell through a closed door and ended up under skyhold where there was this really creepy jack in the box that looked like an angry bird with a top hat and bunny ears and it played creepy music. Then i fell off a cliff and the game froze.

Yeah, I totally found that when I went to Crestwood and immediately mentioned the giant cheese wheel and the skeleton with the scattered cards (or whatever they are) to a friend. I thought it was awesome and told him to look for it behind that building that's locked when you get there - hehehe.
