If you read the loading screen blurbs it talks about Andraste's children. Evidently she had 3 that weren't mentioned much, one of whom always bore daughters. We know from DA:O that Flemeth always bears daughters and that'd also put her in the world long enough ago to wind up the last paladin of Mythal (Until Morrigan) just before Mythal and the other elder gods went pooftie. Since that was also a war with Tevinter, and Flemeth supposedly was married to a Tevinter mage, I'd feel pretty certain saying Flemeth was a daughter of Andraste.
How she knows Solas I dunno, but evidently they were close enough they were hugging buddies and she didn't rip him apart for causing the whole mess in the first place. I also think Solas was 'Old Wolf' like she was 'Mythal' I.E. an agent of, but not the actual deity. What happened when Flemeth 'Died' I have no idea, but I have a feeling she jumped bodies again. She's too paranoid and powerful to let someone gank her like that and Solas was, to be honest, a wimp. Not the sort to bodyjack probably the only real friend he has left. Plus she'd reconciled with Morrigan, even more reason she'd fight to live, not just hug a buddy and then fall over dead without so much as frowning.
