Two Mages make life a lot easier.
I've found it works best with S&B tank, 2 hand warrior, and 2 mages

I'd agree with that as the best combo.
2 blade and shield warriors, 1 barrier + ice mage, and Im controlling dw rogue. play on nightmare. pretty strong combo imo, dont need to worry about deadth too much. plus I deal 80% of the damage anyway. rarely use tactical mode.

My Nightmare Deathsquad in the progess:
Iron Bull as Reaver Tank, devour but no dragon rage. (SnB+Vanguard+Reaver+ Battlemaster 50% stamina regen)
Cassandra as 2H Dps/buffer/debuffer Templar. (Templar + battle master for war horns + stamina regen + some 2H skills)
Solas as support+disabler (Spirit/barrier + Winter tree + clean burn for CC reduction)
Inquisitor DPS Mage (Necro or KE undecided)

His ulti as a tank makes him unkillable, increased damage and attack speed /w lifesteal in addition to devour should be beastly
By using a reaver tank, you gain a tank able to do lots of damage with devour as life sustain instead of the bonus guard

Melee range AOE dispel + AOE stun + Tonns of team buffs and debuffs
Her ulti is team guard, regen and damage resist
Battlemaster horn buffs are glorious

Best party composition is 1 tank + 3 mages. It's also incredibly boring. The more sensible composition is 1 tank + 1 ranged rogue + 2 mages. Any party that has 2 melee characters is not ideal, since the party tends to fare poorly in boss fights, which are usually the toughest parts of the game on Nightmare.
With that being said, I myself am running 1 tank, 1 2H warrior, 1 ranged rogue and 1 mage on Nightmare with minimal tactical mode or pausing. My 2H warrior (respecced Cass deep in Templar abilities) tends to eat up most of the party's healing supplies, but Cass is love, Cass is life.
