Uh...only way to even get Bull's scene to trigger is to hit a heart with him...so...technically the game is right.
You have to click the heart icon to fire up the romance script, so yes, the game is right because the game is written with lusty Inquisitor who only wants casual sex from Bull in mind. Not for an Inquisitor who, say, might want a relationship where sex isn't the begining, middle and end of it. Or have the very first scene begin with casual sex. Some people don't start their relationships like that.
I like Bull as a character and how he is written, and I'm not up in arms over the way his romance is written, it works for some players - just the fact that it leavs veeeeery little room to manouver when it comes to roleplaying. It's his way all the way.

Just to offer a different perspective on this, a lot of the Asian BDSM community don't use safe words at all. They tend to feel that the Top should always be cognizant of the Bottom's state of mind, and to know when "Stop" means "Stop" and when it means "I could use a little more". I'm not Asian myself, but I've been with some Asian women, again on both ends of the relationship To be completely honest, I've done some fairly hardcore stuff and I prefer not having one. I only do it with women I trust, and it adds a sense of uncertainty to things.
Having said all of that, even if you think a safeword is vital, I still don't have any issue with the conversation coming after the act, simply because you have no idea what the actual act is. It's entirely possible it consisted of Bull lightly binding the Inquisitor's hands in front of her, in a way that is easily escapable and more symbolic than anything, and said, "Tell me when you want me to stop". Then, after, when it was decided that things should progress, he wanted to make sure to have the "rules" conversation because he knew things would slowly get more intense.
As for anyone who was disappointed in their two romance options, I'm right there with you. All I've ever wanted is something between Bull and Jack for a F/F romance, but all I ever end up getting is romance. Isabella is my favorite F/F romance, but it's still not really that close to what I'm asking for.
Which was my main gripe with the first sex scene. You get what, three chances to back out, but what the hell is Inquisitor saying yes or no to? All Bull mentions is "riding the Bull" (another horrid line for you) and that Inquisitor doesn't know what that entails. Would he give all those vague warnings just for lightly tying someone to the bed? I doubt it.

I'm going to say that if there was any sort of bondage during the first time. It was very light. Not to mention I can't imagine the Iron Bull continuing if the inquisitor asked him to stop. He's not that kind of person. So I still don't see too big of an issue.

It's a 'breach of protocol' when it comes to the BDSM scene. If you expect someone to sub for you, you tell them exactly what they're signing up for. His being evasive there is bad dom etiquette.
So you think that Bull is into the BDSM scene? Did he visit bdsm clubs in Seheron on a regular basis and signed up on bdsm social networks to discuss how to handle SSC principles with his fellow Qunari?

If the answer to either of these questions is yes, then....Where do I sign up for the Qun? I'll make the no individuality thing work, promise.
Anyways. Safe word is set when the intensity of the scene gets to the point where No doesn't always mean No, and whatever you're doing, or when experimenting with new things, where No might mean a little slower, but the safe word means you've hit some kind of limit and it has to stop, completely.
I really don't think Bull went full out the first time. I think he gave the Inquisitor a pretty damn good preview, and they talked it out from there. Think about some of the possible inquisitors -- what are the chances that a Dalish Elf is going to understand wtf Bull is talking about if he decides to break it down before their first sex scene? It's much easier to just show them, and then talk about it afterwards. (Of course, the Dalish could be a travelling BDSM club for all I know.)
As long as Bull was paying attention, stopped with the Inquisitor said stop, and was careful, it's perfectly acceptable head cannon that their first night was just a sample. I would have loved if Bull had been more forthcoming about what the quizzy was getting into, just because it would have been hot as hell. But it's probably better it was left to our imaginations.

I've also done Dorian's 'Inner Circle' quest, as well as IB's.
